Getting to know Carol de Guzman

I'm so glad you're here — reaching out for support is one of the most courageous steps you can take, and you're in the right place. I'm a board-certified psychiatric mental health nurse practitioner with a background in emergency medicine and a passion for holistic, comprehensive mental health care. I specialize in treating adults with anxiety, depression, burnout, stress-related disorders, and more — through a compassionate, whole-person approach that sees you as far more than a diagnosis. Before becoming a psychiatric provider, I spent years in the Emergency Department, witnessing the devastating impact of untreated mental illness. Those experiences shaped my commitment to proactive, consistent care — helping people regain control before reaching a crisis point. You deserve support before things get to that place. My style is collaborative, warm, and grounded in evidence-based practice. I tailor every care plan to your unique needs, combining medication management with mindfulness-based therapy, lifestyle coaching, and behavioral activation — supporting sleep, nutrition, movement, and emotional regulation. I also evaluate for underlying medical contributors like thyroid imbalances, chronic pain, or medication side effects, because true wellness means looking at the full picture. I offer telehealth services across California — quality care from the comfort of your home. Whether you're feeling stuck, overwhelmed, or ready to make a change, I'm here to listen without judgment and support you every step of the way. You don't have to figure this out alone. Now accepting new patients. Let's take that next step — together.

What should a new client know about working with you?

When you come to me as a new patient, you can expect to feel heard, respected, and never rushed. My approach is warm, collaborative, and judgment-free — your story matters, and I want to understand the full picture of who you are, not just your symptoms. During our first visit, we'll talk about what's been going on, your history, your goals, and what you've tried before. I take a whole-person approach, which means I look beyond just mental health — I also consider lifestyle factors like sleep, nutrition, and movement, as well as potential medical contributors like thyroid issues or chronic pain that can affect how you feel. My practice blends medication management with mindfulness-based therapy, behavioral activation, and lifestyle coaching — because I believe the most effective care is personalized care. If medication is part of your plan, I'll walk you through every decision so you always feel informed and in control. I want our work together to feel like a true partnership. You set the pace, and I'm here to guide, support, and cheer you on. My goal is to help you build real, lasting tools — not just manage symptoms, but genuinely thrive.

What is your typical process for working with clients?

My process begins with a comprehensive intake appointment, where we take the time to really get to know each other. I'll ask about your current symptoms, mental health history, medical history, lifestyle, and goals. This isn't a checklist — it's a conversation. I want to understand not just what's wrong, but what matters most to you and what's getting in the way of living the life you want. From there, I develop a personalized care plan tailored specifically to you. Depending on your needs, this may include medication management, mindfulness-based strategies, behavioral activation, lifestyle coaching, or a combination of approaches. I don't believe in one-size-fits-all care — your plan will evolve as you do. Follow-up appointments typically focus on checking in on how you're feeling, monitoring any medications, adjusting your plan as needed, and working through practical strategies to support your day-to-day wellbeing. I incorporate elements of talk therapy into every visit because I believe the therapeutic relationship is just as important as any prescription. I also take time to screen for underlying medical factors that could be contributing to your symptoms — because sometimes what looks like anxiety or depression has a physical component that deserves attention. Throughout our work together, my goal is to empower you with the tools, insight, and support to take charge of your mental health — for the long term.

I highly recommend her!
My appointment was interrupted by the supervisor of whom I was seeing named, Kim. It was very uncomfortable because clearly they both were suggesting I was cancelling a session, when I was simply asking insurance information. It was very uncomfortable, and to be frank, it was on the brink of HIPPA violations. The practitioner was wonderful, but her supervisor was incredibly unprofessional.
I have only been in one session with Carol so far but she is very helpful, personable and professional! She was very thorough and got my prescription to me quick .

This was one of the most positive mental health visits I’ve had. Carol was kind, attentive, and made me feel safe discussing difficult topics. I never felt rushed, and she took the time to explain treatment options in a way that made sense. I left feeling understood and confident in the plan moving forward. Highly recommend.
Carol is professional, respectful, and kind. She listened to me and helped me create a personlized plan of care. I highly recommend her practice.
I cannot recommend Carol highly enough! She helped me obtain an ESA letter for my dog. She is warm, professional, and incredibly thorough. As a nurse practitioner, she brings both clinical expertise and compassionate understanding to the process. Her empathy truly sets her apart. Carol took the time to really listen to my needs and situation. She ensured that everything was handled with care, discretion, and integrity. The process felt personal, not transactional, and I felt fully supported every step of the way. Her communication was prompt and clear, and the documentation she provided was professionally written, timely, and fully compliant with ESA standards. What impressed me most, though, was her empathy. She clearly understands how important the bond between a person and their emotional support animal can be, and she approaches her work with both heart and skill. If you’re looking for a NP, go to Carol! You’ll be in excellent hands. Thank you, Carol, for everything!
I’d rate my first visit very highly. NP Carol De Guzman was friendly, professional and made me feel safe. She took the time to listen to my concerns. I felt comfortable and well cared for.”
My visit with Carol was smooth and comforting. She has a warm, professional presence that immediately put me at ease. I truly felt heard during our conversation, and it’s clear she’s knowledgeable and confident in her work. I left feeling supported and well taken care of.
I’ve always been hesitant of doing online appointments but I’m glad I still went through with it. I’ve only had one online appointment with Ms Carol so far, but I’ve already felt her empathy even just speaking with her. She was always actively listening to me, had genuine care for me and what I wanted to say. Her suggestions and comments never felt judgmental, which I think is one of the reasons people hesitate to seek mental health care, they fear being judged. Definitely would refer her to more people. Everyone deserves to be seen and cared for by a mental health provider like her.
Carol is a very thorough and caring provider who made me feel seen and heard during my appointment. She was kind and professional. I highly recommend you trust her with your care.
Carol was efficient, empathetic, and a fantastic listener. She asks great questions to understand what is going on and really easy to talk to.
I am beyond grateful for the care I've received from Carol. She takes the time to really listen to me, my concerns, my fears, and my history without rushing through anything. It felt like I was talking to someone who's a friend and who genuinely cared, and that alone makes a huge difference. What I appreciate most is how she combines expertise with compassion. I really felt that Carol wanted to help me. She asked a lot of questions and was very understanding. I felt I was being listened to and that was a great feeling. I'm incredibly thankful for the difference she made in my life.
Carol has been very helpful in addressing my concerns. I have felt very comfortable in expressing what I am going through. And she was very patient in listening to me and thoroughly addressed it to my level of understanding. From that visit, I know she’s the provider I’ve been looking for.
Carol is an exceptional nurse practitioner. She is attentive to her patients needs and is solution driven. Her double board certification as an FNP and PMHNP allows for true comprehensive treatment. I would highly recommend her to anyone seeking mental health needs.
I highly recommend her.
