Getting to know Carol De Guzman

I’m a board-certified psychiatric mental health nurse practitioner with a background in emergency medicine and a passion for holistic, integrative mental health care. I specialize in treating adults with anxiety, depression, burnout, stress-related disorders, and several other mental health conditions—through a compassionate, whole-person approach. Before becoming a psychiatric provider, I spent years working in the Emergency Department. There, I saw the devastating impact of untreated mental illness, including crises involving self-harm and severe emotional distress. These experiences inspired me to provide consistent, proactive psychiatric care—helping people regain control of their lives before reaching a crisis point. My style is collaborative, warm, and grounded in evidence-based practice. I combine medication management with mindfulness-based therapy, lifestyle coaching, and behavioral activation, supporting sleep, nutrition, and emotional regulation. I also carefully evaluate for underlying medical contributors to mental health symptoms—like thyroid imbalances, chronic pain, or medication side effects. Whether you’re feeling stuck, overwhelmed, or simply ready to make a change, I’m here to listen, guide, and support you in building a healthier, more balanced life.

What should a new client know about working with you?

New clients can expect a supportive, nonjudgmental space where you’ll be heard, respected, and actively involved in your care. Our initial evaluation will be 60 minutes to allow time for a thorough understanding of your history, concerns, and goals. Follow-up visits are 30 minutes and typically occur on a monthly basis. I take a collaborative approach—meaning we’ll work together to create a personalized treatment plan that includes medication management, brief therapy, and lifestyle support. My goal is to help you feel empowered, balanced, and hopeful as we work toward long-term wellness together. I’m here to support my patients with thoughtful, collaborative care. Right now, I’m not able to take on new patients who are looking to start or continue medications like stimulants or benzodiazepines, but I’m happy to help guide you to resources that can support you.

What is your typical process for working with clients?

My process begins with a comprehensive 60-minute initial evaluation where I take time to understand your full story—your symptoms, medical history, lifestyle, and goals. From there, we’ll create a personalized treatment plan that may include medication, brief psychotherapy, and strategies like improving sleep, nutrition, stress management, and emotional regulation. Follow-up visits are 30 minutes and typically scheduled monthly to check in, adjust medications if needed, and provide ongoing support. I believe in working collaboratively, so you’ll always be part of the decision-making process. My goal is to create a safe, supportive space where meaningful, lasting change can happen.

Carol is professional, respectful, and kind. She listened to me and helped me create a personlized plan of care. I highly recommend her practice.
This was one of the most positive mental health visits I’ve had. Carol was kind, attentive, and made me feel safe discussing difficult topics. I never felt rushed, and she took the time to explain treatment options in a way that made sense. I left feeling understood and confident in the plan moving forward. Highly recommend.
I cannot recommend Carol highly enough! She helped me obtain an ESA letter for my dog. She is warm, professional, and incredibly thorough. As a nurse practitioner, she brings both clinical expertise and compassionate understanding to the process. Her empathy truly sets her apart. Carol took the time to really listen to my needs and situation. She ensured that everything was handled with care, discretion, and integrity. The process felt personal, not transactional, and I felt fully supported every step of the way. Her communication was prompt and clear, and the documentation she provided was professionally written, timely, and fully compliant with ESA standards. What impressed me most, though, was her empathy. She clearly understands how important the bond between a person and their emotional support animal can be, and she approaches her work with both heart and skill. If you’re looking for a NP, go to Carol! You’ll be in excellent hands. Thank you, Carol, for everything!
I’d rate my first visit very highly. NP Carol De Guzman was friendly, professional and made me feel safe. She took the time to listen to my concerns. I felt comfortable and well cared for.”
Carol is a very thorough and caring provider who made me feel seen and heard during my appointment. She was kind and professional. I highly recommend you trust her with your care.
I’ve always been hesitant of doing online appointments but I’m glad I still went through with it. I’ve only had one online appointment with Ms Carol so far, but I’ve already felt her empathy even just speaking with her. She was always actively listening to me, had genuine care for me and what I wanted to say. Her suggestions and comments never felt judgmental, which I think is one of the reasons people hesitate to seek mental health care, they fear being judged. Definitely would refer her to more people. Everyone deserves to be seen and cared for by a mental health provider like her.
My visit with Carol was smooth and comforting. She has a warm, professional presence that immediately put me at ease. I truly felt heard during our conversation, and it’s clear she’s knowledgeable and confident in her work. I left feeling supported and well taken care of.
Carol was efficient, empathetic, and a fantastic listener. She asks great questions to understand what is going on and really easy to talk to.
I am beyond grateful for the care I've received from Carol. She takes the time to really listen to me, my concerns, my fears, and my history without rushing through anything. It felt like I was talking to someone who's a friend and who genuinely cared, and that alone makes a huge difference. What I appreciate most is how she combines expertise with compassion. I really felt that Carol wanted to help me. She asked a lot of questions and was very understanding. I felt I was being listened to and that was a great feeling. I'm incredibly thankful for the difference she made in my life.
Carol has been very helpful in addressing my concerns. I have felt very comfortable in expressing what I am going through. And she was very patient in listening to me and thoroughly addressed it to my level of understanding. From that visit, I know she’s the provider I’ve been looking for.
Carol is an exceptional nurse practitioner. She is attentive to her patients needs and is solution driven. Her double board certification as an FNP and PMHNP allows for true comprehensive treatment. I would highly recommend her to anyone seeking mental health needs.
I highly recommend her.
