Dr. Daniel Irby, MD

Psychiatrist
CA
As a board-certified psychiatrist, I focus my practice on medication management for a broad range of mental health conditions. I care about seeing the full picture of who you are — your background, your current struggles, and what you want your life to look like — and I use that understanding to shape a treatment plan alongside you. Psychiatry works best as a partnership, so I make a point of walking you through the thinking behind every recommendation, giving you what you need to make confident choices about your care. My patients include adolescents, transitional-age youth, and adults. I practice careful, evidence-informed psychopharmacology for concerns such as depression, anxiety, OCD, and related conditions. Getting the medication and dosage right is rarely one-size-fits-all, so I track how you're responding, weigh benefits against side effects, and refine the plan as your situation evolves. When standard medications haven't brought enough improvement, I also provide interventional treatments. These include TMS (transcranial magnetic stimulation) for depression and OCD, along with esketamine and IV ketamine for treatment-resistant depression. If it seems relevant, I'm happy to talk through whether one of these might strengthen your overall plan. I earned my MD through the UC Berkeley–UCSF Joint Medical Program and completed psychiatry residency in San Mateo County. My career has been rooted in outpatient psychiatry, with particular depth in psychopharmacology and interventional care, and I keep up with advances in the field so my patients can benefit from current, well-supported approaches. I'm also fluent in Portuguese and speak intermediate Spanish. Whether you're living with a long-standing condition, wanting a fresh evaluation, or seeking a second opinion on medications you're already taking, you can count on an approach that is thorough, respectful, and centered on your individual needs. I look forward to working with you.

What should a new client know about working with you?

Please note that I don't accept EAP. With that in mind, my first priority when we work together is to know you as a whole person rather than a diagnosis. Our first visit is really a conversation: I'll ask about your history, what you're experiencing now, the goals that brought you in, and which treatments have or haven't helped you before. I want to hear things in your own words, not run through a checklist, because that's what allows me to build a medication plan around your actual life instead of symptoms alone. My practice centers on medication management, using an evidence-informed and collaborative approach to psychopharmacology for conditions like depression, anxiety, and OCD. I think you deserve to understand the reasoning behind each recommendation, so I'll walk you through why a given medication makes sense, what improvements to look for, how long it typically takes to work, and which side effects to keep an eye on. We make decisions as a team — I bring the expertise and the options, and you decide what feels right for you. Careful follow-up matters to me. Since finding the right medication and dose is usually a process, I'll keep checking in on how you're responding and make adjustments thoughtfully over time. When standard medications don't provide enough relief, we can also consider interventional options, including TMS (transcranial magnetic stimulation) for depression and OCD, and esketamine or IV ketamine for treatment-resistant depression. You can count on me to be thorough, respectful, and unhurried. I encourage your questions and want you to feel at ease being honest about how you're truly doing, including any worries about medications. I care for adolescents, transitional-age youth, and adults, and I'm fluent in Portuguese with intermediate Spanish. Whether you're beginning treatment for the first time, managing a long-standing condition, or looking for a fresh perspective on your current medications, I look forward to working with you.

What is your typical process for working with clients?

Please take note that I don't accept EAP programs. Our work begins with a comprehensive psychiatric evaluation, typically our longest appointment together. During this visit, I'll explore your current concerns and symptoms, your mental health and medical background, any medications you've tried and how they affected you, your family history, and the goals you'd like to reach. I also want a sense of your everyday life and the things that matter most to you, because effective treatment has to suit the individual rather than just the diagnosis. From there, I'll share my clinical impressions, we'll review the available options, and we'll settle on a starting plan together. While my practice is built around medication management, our appointments are never a rushed prescription hand-off. I make time to explain each recommendation — why a specific medication fits, what results to anticipate, how long it may take to take effect, and what side effects deserve attention — so your decisions are well informed. My part is to provide expertise and options; yours is to choose what feels comfortable to you. Follow-up appointments center on how you're genuinely doing: whether the medication is helping, how well you tolerate it, and what changes could improve your outcome. Because identifying the right medication and dose is usually a gradual process, I monitor closely and make adjustments with care rather than haste. Visits are generally more frequent early on and can be spaced out once you feel stable. For patients who haven't gained enough relief from standard medications, I can also explore interventional treatments such as TMS (transcranial magnetic stimulation) for depression and OCD, or esketamine and IV ketamine for treatment-resistant depression. Above all, my approach is collaborative, respectful, and unhurried. I welcome your questions and want you to feel free to be honest about how you're really doing, so we can adjust course together as your needs evolve.
