About Dr. Farhad Nassiri Afshar
PPO-only clinic (Anthem Blue Cross, Aetna, Cigna, UnitedHealthcare) and Original Medicare—not in network with Blue Shield of California or HMO plans—in Irvine, led by a board-certified internal medicine physician providing adult primary care in Orange County. We have a 24h cancellation policy with a $60 late-cancel/no-show fee.
I’m Farhad Nassiri Afshar, MD (Dr. Farhad Afshar), an ABIM-certified internist and founder of Afshar Medical Group. I help adults prevent illness and manage chronic conditions with clear, practical plans.
Clinical focus: high blood pressure, diabetes, high cholesterol, thyroid disease, weight issues, asthma/allergies, digestive and sleep problems, and preventive/annual physicals. I also treat common acute issues like colds, infections, UTIs, rashes, minor injuries, and medication questions.
Access & convenience: in-person visits in Irvine and secure video visits for adults anywhere in California. New and existing patients are welcome, with timely follow-up and coordinated referrals.
Insurance:
We are a PPO + Original Medicare clinic and not in network with Blue Shield of California. In-network plans include Anthem Blue Cross PPO, Aetna PPO, Cigna PPO, UnitedHealthcare (UHC) PPO, and Original (“classic”) Medicare with the red-white-blue card. We are not in-network with Blue Shield, Medicare Advantage/SCAN-type plans, Medi-Cal/Medicaid, or HMO/EPO/“High Performance”/BlueHPN-type products. If your card says HMO, EPO, HPN/BlueHPN, Select, or lists a medical group/IPA, we are usually out of network and visits may be self-pay. Self-pay visits are $150–$250, due at the visit.
Office policies:
• Confirm within 24h of booking and finish forms and ID/insurance 24 business hrs before.
• Cancel within 24h of booking with no fee; after that, cancel or no-show with less than 24 business hrs notice has a $60 fee.
• Televisits: you must be in California, forms and ID/insurance done, and copay or self-pay paid 20 min before.

Dr Afshar gently checked my heart and lung sound with his Stethoscope, palpate and checked my abdomen also my lower extremities, gave his advise and recommendation in a manner you easily understand. A very simple and professional MD highly recommended.

Dr. Afshar is amazing! I booked on a Friday the day after Christmas and he said he normally closes on that day but I had already booked so he wanted to honor that. Not only did he show up and do all the work himself he did a phenomenal job. It’s obvious that the man is passionate about his work and cares about his patients. After some bad experiences with doctors in the past that felt like they half-assed their practices in the past this was refreshing to see. Could not recommend a doctor more!
