I’m Farhad Nassiri Afshar, MD, an ABIM-certified internist and founder of Afshar Medical Group. I provide clear, practical, prevention-focused care for adults, including preventive care, in-person annual physicals, and long-term management of common medical conditions.
Common areas of care include high blood pressure, diabetes, high cholesterol, thyroid disease, asthma/allergies, digestive concerns, sleep concerns, and other routine adult primary care needs.
Insurance:
We are a PPO-only clinic. In-network plans include Anthem Blue Cross PPO, Aetna PPO, Cigna PPO, and UnitedHealthcare PPO. We are not in network with Blue Shield of California or HMO plans. If your insurance card says HMO, EPO, Select, BlueHPN/HPN, or lists a medical group/IPA, we are generally out of network and visits may be self-pay. Self-pay visits typically range from $150–$250 depending on visit type and are due at the time of service. For many high-deductible PPO plans, problem-focused visits may be applied to the deductible and can still result in out-of-pocket costs even when we are in network. Please verify your exact plan before booking.
Office policies:
Please confirm your appointment, complete forms, and upload your ID and insurance information within 24 hours of booking. Our office may contact you to confirm insurance or appointment details. If we do not receive a response, we may release the appointment. There is no cancellation fee during the first 24 hours after booking. After that 24-hour grace period, cancellations and no-shows may be subject to a $60 fee.

I am a new patient. The receptionist was amazing. He knew who I was before I could even take out my insurance card. Dr Afshar saw me within minutes of my arrival and was very thorough with background inquiries and my overall health condition before we talked about my current issue. I think he cared about MY concerns and communicated a clear plan to address those concerns. I will return in a week to start the plan.

Dr Afshar gently checked my heart and lung sound with his Stethoscope, palpate and checked my abdomen also my lower extremities, gave his advise and recommendation in a manner you easily understand. A very simple and professional MD highly recommended.

Dr. Afshar is amazing! I booked on a Friday the day after Christmas and he said he normally closes on that day but I had already booked so he wanted to honor that. Not only did he show up and do all the work himself he did a phenomenal job. It’s obvious that the man is passionate about his work and cares about his patients. After some bad experiences with doctors in the past that felt like they half-assed their practices in the past this was refreshing to see. Could not recommend a doctor more!
