What should a new client know about working with you?

As a general adult psychiatrist and combat veteran, I have spent more than twenty- two years helping people across the Dallas–Fort Worth area recover from depression, PTSD, anxiety, and substance use disorders. I founded Hawkins Psychiatry in 2004 after completing my residency at John Peter Smith Hospital and serving as an Army Reserve psychiatrist in Kosovo and Germany, where I treated soldiers under combat stress and screened thousands returning from Iraq for PTSD and traumatic brain injury. That work taught me that many patients do not respond to medication alone, and that they deserve more than another prescription trial. In 2020 I opened the TMS Depression Center of Mansfield to bring evidence-based alternatives directly into my practice. I offer repetitive transcranial magnetic stimulation (rTMS), Spravato (esketamine) for treatment-resistant depression, and micro current neurofeedback. These are non-narcotic, non-sedating options for patients who have exhausted conventional treatment. I also use DNA pharmacogenetic testing to guide medication selection and reduce trial and error. My goal is for every patient gets a clear explanation of the full range of options available to them, and a real chance to feel like themselves again.

What is your typical process for working with clients?

Assessing for treatment-resistant depression. A central part of follow-up is determining whether your depression meets criteria for treatment resistance. That generally means an inadequate response to two or more antidepressant trials at an adequate dose and for an adequate duration. I I also reassess the diagnosis itself, since bipolar spectrum illness, undertreated anxiety, trauma, ADHD, thyroid disease, sleep apnea, and substance use can all masquerade as resistant depression. TMS and Spravato as primary options. If you do meet criteria for treatment-resistant depression, my first line of discussion is interventional treatment — transcranial magnetic stimulation (TMS) or Spravato (esketamine) — rather than another round of trial-and-error with oral medication. Both have strong evidence in this population and work through mechanisms different from standard antidepressants. TMS is non-systemic and non-sedating, involves no medication and no anesthesia, and is delivered in a series of outpatient sessions you can drive yourself to and from. Spravato is a nasal spray given in a certified treatment center alongside an oral antidepressant, with a monitoring period after each dose, and can work more rapidly. I’ll walk you through what each involves week to week, the side effects, the insurance and prior-authorization realities, and which one fits your history, schedule, and preferences
