What is your typical process for working with clients?

What we need from you... ANY DOCUMENTATION FROM YOUR PRIOR PROVIDER If you have any prior documents provide a release of information form to your provider or hospitalization documents. MOST RECENT LAB WORK Please provide your moors recent labs obtained if they were within the last year. If you have not received any recent lab work we will provide an order of labs to be conducted within the first couple of weeks after the initial evaluation. MOST RECENT BRAIN IMAGES If you have obtain any CT or MRI/fMRI scan of your brain please provide the CDs or and report from the radiologist. INITIAL VISIT The initial evaluation, along with any psychotherapy or medication management that has been ordered. 2 week courtesy call Dr. HECTOR will reach out to you to check out on the status of any pending information/ medication side effects 1st follow up First follow up evaluation, supportive psychotherapy and medication management. Schedule additional follow ups depending on need.

Initial session: $350 for 60 minutes

    • We are a self-pay practice. We accept cash and credit cards for payment. • Payment is due at the time of service and/or before appointment is confirmed 48 hours before. Your appointment is dedicated to you and your concerns. • Initial 60-minute evaluation includes No Rush appointments require extensive research, full mental status exam, lab interpretation and planning by the mental health team and psychiatrist are billed before the scheduled appointment. Cancellation requests MUST be made at least 48 hours in advance of your appointment. • 30 minute problem focused visits or telephone consultations not included in your program plan are billed before scheduled appointment. • Payment is due at the time services are rendered. • We reserve the right to charge a $50.00 missed appointment fee for no- show visits. • Late arrivals to appointments.
