Dr. Jacob Gutierrez, MD

Dr. Jacob Gutierrez, MD

Psychiatrist
CA
I'm Dr. Jacob J. Gutierrez, MD, and I bring a double board certification in General Psychiatry and Addiction Psychiatry to my telepsychiatry practice, The Clear Practice. My path to this field started with a simple curiosity about people — wanting to understand the whole person, not just their symptoms. That curiosity led me to specialize further in addiction psychiatry, working alongside people navigating some of the hardest chapters of their lives as they find their way to recovery. My approach is built on transparency and collaboration. I'm always straightforward with you about what I know, what I don't, and what falls inside or outside my scope — no vague explanations. When I recommend a treatment plan, I walk you through my thinking so you understand not just what I'm suggesting, but why. Your goals and your life circumstances shape that plan; we build it together, as a team. I provide psychiatric evaluation and medication management for depression, anxiety, PTSD, and ADHD, along with Medication-Assisted Treatment (MAT) for opioid and alcohol use disorders, including buprenorphine/Suboxone management. I also treat dual diagnosis, where substance use and psychiatric symptoms overlap. I offer compassionate, non-judgmental care to every patient, regardless of background or history — a space where you can show up honestly. This practice currently operates on a self-pay basis, with superbills available upon request for out-of-network reimbursement. I see adults located in California, via telehealth only.

Getting to know Dr. Jacob Gutierrez

What should a new client know about working with you?

A few things that might be helpful to know before your first visit. I'm direct. If something falls outside my scope, if a medication isn't the right fit, or if I think a different level of care would serve you better, I'll tell you plainly rather than let you find out later. That same honesty goes both ways — I'll ask you to be upfront with me too, especially around substance use, since that information helps me keep your treatment safe rather than something to hide. You won't get vague explanations here. I walk you through my clinical reasoning at every step, so you always understand not just what I'm recommending, but why. You're an active partner in your care, not someone treatment happens to. This is a judgment-free space. Whatever your background, lifestyle, or history, you're welcome here exactly as you are. That matters especially for patients navigating substance use alongside psychiatric symptoms — shame keeps people from getting help, and I want this to be a place where you don't have to manage anyone else's discomfort about your story. Practically: I'm telehealth-only, seeing adults physically located in California at the time of appointments. This practice currently operates on a self-pay basis, with superbills available upon request for out-of-network reimbursement. If any of that resonates, I'd be glad to work with you.

What is your typical process for working with clients?

My process starts with listening. At our first visit, I take the time to understand your full history — not just your symptoms, but your goals, your life circumstances, and what's brought you to seek care now. From there, I walk you through my clinical thinking so you understand not just what I'm recommending, but why. Nothing happens without you understanding the reasoning behind it. Together, we build a treatment plan that fits your actual life, not a one-size-fits-all template. If medication is part of that plan, I explain the options, the rationale, and what to expect, and we decide together which approach makes sense for you. For patients navigating substance use alongside psychiatric symptoms, I integrate Medication-Assisted Treatment directly into that same collaborative process. Follow-up visits are where we track progress, adjust as needed, and keep refining the plan based on what's actually working for you. I'm always honest about what's improving, what isn't, and when something outside my scope calls for a referral — I'd rather point you toward the right resource than keep you somewhere that isn't serving you. Throughout care, I aim to be direct, transparent, and non-judgmental. You should always know where you stand with your treatment and feel like an active partner in it, not a passive recipient. My goal is for you to leave every visit with more clarity than you came in with.
