About Jerise Agha

I am Jerise Agha, MSN, AG-ACNP-BC, a board-certified Adult-Gerontology Acute Care Nurse Practitioner and the founder of Tikari in Middletown, Delaware. My practice focuses on three areas: wound care, chronic disease management, and transitional care after a hospital or rehab stay. I treat diabetic foot ulcers, venous leg ulcers, pressure injuries, and slow-healing surgical wounds. I also help patients manage diabetes, high blood pressure, heart failure, COPD, and kidney disease. My background in acute and critical care helps me recognize problems early and keep patients safely at home. I see patients in my office and through home visits for those who find it hard to get out. Every visit is with me, not a rotating team. I take time to explain the plan, involve family and caregivers, and coordinate with your primary care provider, specialists, and home health agency.
