Adult Psychiatric & Mental Health Nurse Practitioner
FL
I am Jose Gabriel Chavez, a dual board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and Adult-Gerontology Acute Care Nurse Practitioner (AGACNP-BC). With extensive experience caring for complex patients in hospital settings, I founded JNP Mental Health to provide accessible, compassionate, and truly integrated psychiatric care through secure telehealth across Florida. My journey in healthcare began with a strong foundation in critical care, nephrology, and adult-gerontology medicine. This background has given me a unique understanding of how mental health conditions interact with chronic medical issues such as diabetes, heart disease, kidney concerns, and hypertension. I am passionate about whole-person care because I have seen how addressing both mind and body leads to better outcomes and improved quality of life. At JNP Mental Health, our philosophy is simple yet powerful: we believe that with the right support, every individual can heal, rise above their challenges, and thrive in their daily lives. I specialize in treating adults experiencing anxiety, depression, ADHD, mood disorders, and the mental health aspects of living with chronic conditions. My dual certification allows me to offer a level of integrated care that many patients find is missing in traditional settings. I practice with a collaborative, respectful, and evidence-based approach. I speak both English and Spanish fluently, which enables me to provide culturally sensitive care to the diverse communities of South Florida. All services are delivered via secure video telehealth, making high-quality psychiatric care convenient, private, and flexible. Whether you are seeking care for the first time, transitioning from hospital treatment, or looking for a provider who truly understands the connection between mental and physical health, I am here to support you. My goal is to create a safe, non-judgmental space where you feel heard and empowered to reach your personal goals. I

What should a new client know about working with you?

Working with me means entering a collaborative, respectful, and highly individualized therapeutic relationship. I view every client as the expert on their own life. My role is to bring clinical expertise, evidence-based tools, and genuine compassion to help you achieve meaningful progress. My style is warm, direct, and empowering. I strive to create a safe, non-judgmental environment where you can speak openly about your experiences. I listen carefully, ask thoughtful questions, and work together with you to develop realistic and achievable treatment plans. Progress is measured not only by reduction in symptoms but also by improvements in daily functioning, relationships, sleep, energy, and overall quality of life. I take an integrated whole-person approach that considers both mental health symptoms and physical well-being. This is especially valuable for individuals managing anxiety, depression, ADHD, or mood disorders alongside chronic conditions such as diabetes, heart disease, or kidney issues. Treatment often combines medication management when appropriate with supportive psychotherapy, psychoeducation, practical coping strategies, and lifestyle recommendations. I explain all recommendations in understandable language and encourage questions at every step. My dual certification in psychiatry and adult-gerontology care allows me to bridge gaps that often exist when mental and physical health are treated separately. I am bilingual in English and Spanish and committed to providing culturally sensitive care. Whether this is your first experience with psychiatric services or you are seeking a more coordinated and personalized approach, I am dedicated to tailoring care to your unique needs, values, and goals. At JNP Mental Health, you are not just another appointment. You are a partner in your healing process. I am committed to walking alongside you as you heal, rise, and thrive.

What is your typical process for working with clients?

The process of working together at JNP Mental Health is designed to be clear, supportive, and fully individualized from the very beginning. It starts with a comprehensive initial psychiatric evaluation, typically lasting 60 minutes. During this thorough session, we will explore your current concerns, symptoms, full medical and psychiatric history, current medications, and personal goals in detail. I will also assess how mental health symptoms may be connected to any chronic medical conditions you are managing. This allows me to gain a complete understanding of your situation and begin forming an effective treatment plan. After the initial evaluation, we will discuss my clinical recommendations and work together to create a personalized plan that aligns with your needs and preferences. This plan may include medication management, supportive psychotherapy, lifestyle modifications, lab testing, or coordination with other healthcare providers. Follow-up appointments are usually scheduled every two to four weeks initially, depending on your specific situation. These visits typically last 20-45 minutes and focus on reviewing your progress, monitoring medication effectiveness and side effects, adjusting the treatment plan as needed, developing coping skills, and supporting overall stability and wellness. Many clients benefit from combining medication management with supportive therapy during the same visit. All appointments are conducted through secure video telehealth, providing convenience and privacy. I offer flexible scheduling, including some weekend availability, to better accommodate busy lives and work commitments. Throughout our work together, I emphasize clear communication, education, and shared decision-making. You will always understand the reasoning behind each recommendation and will be actively involved in every step of your care. My goal is not only to help reduce symptoms but also to build long-term resilience so you can heal, rise, and thrive.
