Hi, I’m Lina Wilches-Karrat, a bilingual psychiatric nurse practitioner with over 19 years of experience in the nursing field. I completed my advanced psychiatric training at Walden University, where I focused on treating mood disorders, attention disorders (including ADHD), and supporting children and teens through their mental health journeys. My approach to care is warm, open-minded, and collaborative. I believe in creating a space where patients feel truly heard and respected. I incorporate elements of cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), motivational interviewing, and behavioral therapy into my sessions, which primarily focus on medication management. While I am not a therapist, I believe in blending therapeutic techniques into each visit to ensure a well-rounded experience. I DO NOT SEE PATIENTS FOR PSYCHOTHERAPY WITHOUT MEDICATION MANAGEMENT AS ANY THERAPY INCORPORATED INTO OUR SESSIONS IS DONE ALONGSIDE MEDICATION MANAGEMENT. I work with individuals dealing with conditions such as depression, bipolar disorder, anxiety, ADHD, and more. I’m especially passionate about helping children and teens thrive by addressing their challenges early on and giving them the tools to succeed. I also offer services like lab work, genetic testing, and supplement guidance when appropriate, and I'm always learning about new approaches in psychiatry to better support my patients. Outside of work, I enjoy spending time with my family, training my new puppy, and exploring photography and hidden gems around South Florida. What should a new client know about working with you?

When working with me, you can expect a compassionate, structured, and collaborative approach that puts your well-being at the center of care. My primary focus is on medication management, and I take time to understand the full picture—your background, symptoms, life circumstances, and goals—before we make any treatment decisions. During our initial session, we’ll work together to develop a comprehensive and personalized treatment plan that aligns with your values and preferences. This may include not only medication, but also lab work to rule out organic causes of mental health symptoms, genetic testing, and targeted supplementation when appropriate. While I may incorporate elements of supportive psychotherapy or tools like CBT into our sessions, I do not provide talk therapy. Instead, sessions are designed to monitor your progress, discuss side effects, adjust your medications if needed, and ensure that your treatment is moving in the right direction. I also welcome and encourage collaboration with your therapist or other healthcare providers to ensure that your care is well-coordinated and effective. My goal is never to push medications but to work with you to find what feels right—whether that means a holistic route or pharmacological support. I value openness, trust, and empowering patients to make informed choices. I work with children, teens, and adults, and I take pride in creating a space where all my patients feel heard and respected. Please be aware that no-show fees apply to help cover the cost of reserved time. The fee for missed initial visits is $175 and $95 for missed follow-up sessions. These fees are not punitive, but necessary to ensure time and resources are used appropriately. No-show fees may be escalated to collections if not resolved. We ask that all cancellations or reschedules be made at least 24 hours in advance. I’m here to support you with warmth, clinical knowledge, and a commitment to finding what works best for you.

What is your typical process for working with clients?

In our first session together, I take time to really get to know you. We’ll review your personal and family history, current symptoms, previous treatments, and your overall health. I may ask questions about your upbringing, life stressors, and goals, so I can better understand the full picture of what’s going on. You’ll also complete 10–20 minutes of intake paperwork ahead of time, which helps us spend more of our session focusing on developing a thoughtful and individualized treatment plan rather than screening forms. If appropriate, I may order lab work or genetic testing to explore any medical or biological factors that could be impacting your mental health. I work with both children and adults, and I tailor each session based on age, clinical needs, and comfort level. While I am not a therapist and do not offer formal talk therapy, I do include elements of supportive conversation and evidence-based strategies (like CBT or behavioral techniques) within medication management appointments when helpful. In follow-up sessions, we’ll check in on how you're responding to medications, make any necessary adjustments, and continue building a plan that feels right for you. I strive to create a judgment-free, collaborative space, and I’ll always include you in decisions about your care. If you’re working with a therapist, I’m happy to collaborate to ensure we’re aligned on your diagnosis and treatment.
