This is a CASH-PAY practice ONLY. Insurance is not billed.
Referral Program:
As a thank you, patients may receive a $25 credit per referral toward future care when a referred patient completes their visit.
Consultation Fees:
New Patient Consultation: $99 (applied toward treatment plan if initiated)
Monthly Programs (no separate visit fee):
Weight Management: $169–$299/month
(visits, medications, shipping, supplies included)
Hormone Replacement Therapy: $149-$269/month
Testosterone Replacement Therapy: $199/month
(For HRT/TRT- visits, medications, shipping, supplies and followup labs included)
Fertility Optimization: $199/month
For patients not enrolling in a program, individualized care plans and follow-up visits are available.
