Just a quick note: We’re a self-pay practice and do not bill insurance for appointments. Our practice operates outside of traditional insurance.
A one-time only consultation is $250. But if you’re looking for ongoing, more personalized care, many patients find our membership options to be a great fit—it’s $199/month with a one-time registration fee of $49.

Culver Primary is a direct primary care practice providing a more personalized, patient-centered approach to healthcare. We offer a membership model designed for ages 14 – 65+.
Our membership offers simple, transparent pricing with no surprise fees:
Ages 14–17 membership: $169/month
Ages 18+ membership: $199/month
Ages 18+ duo membership: $350/month
For non-members, consultations are available for $250, with additional charges for any diagnostic or lab services as needed.
