No communication with the office, everything was done online and when they couldn't confirm insurance coverage, they simply canceled the appointment. No one called me, everything was done through email. I am 89 and it's a challenge to do things online. So since there was no one to talk to, I just didn't understand what else I needed to provide in addition to my insurance card.
