Insurance is not accepted. payment and intake forms must be completed within 3 hours of booking, including submission of a valid photo ID. Appointments not completed within this timeframe may be released. Please note: This practice does not provide controlled substance prescribing (including opioids, stimulants, or benzodiazepines), ongoing psychiatric care, ADHD management, or disability and clearance evaluations.
