I have completed one of these survey forms already. I was not examined at all. I still need to see a physician because this was supposed to be my annual and I did not receive any significant service. I need to reserve the money that would have paid her to see someone who cares.

I booked an appointment via Zocdoc. I received the confirmation for my appointment and it stated the medical treatment I needed. I was directed to an online site to complete a lengthy series of forms. It took one hour of my time on Easter Sunday away from my family & loved ones. But i did it because I Need the treatment. When arrived the secretary gave me a lengthy set of forms to fill out. I remarked that i filled out these same forms online and the secretary said they maintain their own forms separAte from PRIVIA. I completed the forms some of WHICH required my social security number. PRIVIA did not ask for it. But dr sally mcfarland’s forms require it. I saw the nurse and she asked me why I was there. I explained. She said “we don’t do that”. And I was dismissed. WHAT A MONUMENTAL WASTE OF MY TIME. SEEMED TO BE AN INFORMATION EXTRACTION PROCESS. Now thd doctors staff and the dr herself have all of my personal data. Nice work. Identity theft starts Monday.
