Elevate Wellness offers convenient, affordable telemedicine visits for acute care needs, as well as a Direct Primary Care (DPC) membership for those looking for ongoing, personalized care.
Acute Care Telemedicine Visits:
$75 per visit (30 minutes)
Ideal for concerns such as respiratory infections, UTIs, rashes, medication refills, and other common acute issues.
Looking for ongoing care?
Elevate Wellness also offers a Direct Primary Care (DPC) membership. This means no insurance billing—just a simple, affordable monthly fee for personalized, accessible care.
Membership Options:
Telemedicine Only (Adult): $50/month
Telemedicine Only (Child ages 2–17): $25/month
What’s included:
Unlimited telemedicine visits
Direct messaging access
Same/next-day appointments
Support for acute, chronic, weight management, and hormone health needs
Getting Started:
If you’re interested in membership after your visit, your enrollment fee will be waived and you can continue with the $50/month membership starting the same date the following month.
