No show/not canceled appointment by Friday 12PM fee schedule:
1st - waived
2nd - $50 paid prior to seeing the doctor for next (3rd) appointment.
3rd onwards - $75. paid prior to seeing the doctor for next appointment.
Appointment cancelation must be done by Friday 12PM prior to your Saturday appointment If not canceled, there is a cancelation fee of $50.
Valid Insurance and ID cards are required prior to seeing the doctor.
Patients are responsible to inform the clinic if they require prior authorization. If fail and insurance company rejects the bill, the patient/financially responsible party will be billed for the same.

The doctor was great and very friendly. It was hard to find the office building, as it’s not clearly stated where exactly it was. Zocdoc ended up scheduling me for an appointment time that was actually not available. The office called us at our scheduled appointment time and let me know this, but i was able to get another appointment the same day. The doctor and receptionist were both very friendly.
