Dr. Takiyah Jones, DNP

Nurse Practitioner (Psychiatry)
NY
Seeking mental health care can feel overwhelming, but you don’t have to navigate it alone. My goal is to create a welcoming, judgment-free space where you feel heard, understood, and actively involved in your care. I hold a Doctor of Nursing Practice (DNP) degree and am dual board-certified as both a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and Family Nurse Practitioner (FNP-C). I founded Continuum Integrated Mental Health because I believe effective care begins with understanding the whole person—not just a diagnosis. I work with adults experiencing anxiety, depression, ADHD, trauma-related concerns, bipolar disorder, OCD, insomnia, burnout, stress, grief, relationship challenges, and major life transitions. Every treatment plan is personalized to your goals, values, and unique circumstances. Appointments include comprehensive psychiatric evaluations, medication management, supportive psychotherapy, and education to help you understand your diagnosis and treatment options. I believe the best decisions are made when patients feel informed, comfortable asking questions, and actively involved in developing their care plan. My experience across emergency medicine, primary care, community mental health, hospice, corrections, and outpatient psychiatry has reinforced one belief: healing isn’t one-size-fits-all. Whether you’re seeking care for the first time or looking for a fresh perspective, I look forward to partnering with you on your journey toward improved mental health and well-being.

What should a new client know about working with you?

My style is warm, collaborative, and conversational, while still providing enough structure to keep us focused on your goals. The initial visit includes detailed questions about your symptoms, health, medications, sleep, relationships, and life experiences. You are always welcome to pause, ask questions, or tell me when something does not feel right. My approach is supportive, trauma-informed, and influenced by cognitive behavioral therapy. Together, we explore how thoughts, emotions, behaviors, relationships, and physical health may be connected. I may offer practical strategies, ask reflective questions, gently challenge unhelpful patterns, or suggest skills to practice between visits. You can expect an affirming, nonjudgmental environment where we work at a pace that respects your comfort while continuing to make meaningful progress.

What is your typical process for working with clients?

Care begins with a comprehensive psychiatric evaluation. I review your current symptoms, psychiatric and medical history, previous medication trials, sleep, functioning, safety, and treatment goals. When helpful, I may use screening questionnaires, review previous records, or recommend laboratory testing. We conclude by discussing diagnostic impressions and recommended next steps. Follow-up visits focus on symptom changes, medication effectiveness, side effects, daily functioning, and progress since the previous appointment. Medication adjustments and monitoring are completed when appropriate. I also provide talk therapy. Some visits incorporate brief supportive or CBT-informed interventions alongside medication management, while dedicated psychotherapy visits may be scheduled for more in-depth work.

I accept select commercial insurance plans. Coverage varies by state and specific plan, so eligibility and benefits must be verified before your first appointment. Depending on your plan, insurance billing and patient payments may be processed through Headway or Alma. Copays, coinsurance, and deductibles are determined by your insurance plan. Self-pay is also available. For patients using out-of-network benefits, I can provide a superbill to submit to your insurance company for possible reimbursement.
