Dr. Poprawski earned her medical degree from the Medical University of Silesia in Katowice, Faculty of Medical Sciences, and completed her residency in psychiatry at Loyola University Medical Center. Having developed and successfully applied a proprietary method of medication selection, Dr. Poprawski nonetheless recognizes that this cutting-edge technology is highly effective in the treatment of psychiatric conditions, especially in cases where traditional methods have failed or provided an inadequate level of relief.

About Relief Mental Health
Headquartered in Oak Brook, Illinois, Relief Mental Health is an outpatient provider of transcranial magnetic stimulation (TMS), psychedelics (Spravato esketamine nasal spray), psychiatric medication management, and talk therapy, for the treatment of depression, obsessive compulsive disorder, anxiety and other mental health diagnoses. Founded in February 2020, Relief has 10 locations throughout Chicagoland, in Wisconsin and New Jersey. In Illinois, clinics are located in Chicago (Lakeview and West Loop), Oak Brook, Orland Park, Northbrook, Rockford, and St. Charles. Relief’s other clinics are in Warren and Red Bank, New Jersey, and West Allis, Wisconsin.
What is your typical process for working with clients?
Patients will receive an intake form upon booking; this intake form must be completed and submitted at least 24 hours prior to your appointment in order for Relief Mental Health to confirm or cancel your appointment in time. If confirmed, patients will receive additional new patient paperwork via email that is required to be completed and submitted prior to your appointment as well. Please note that Relief Mental Health is in-network with Medicare and most insurance providers, including Aetna, BlueCross BlueShield, Cigna, Humana, Tricare and United Healthcare commercial PPO policies. Relief Mental Health is out-of-network with Medicaid and commercial HMO policies.

INTAKE FORM & NEW PATIENT PAPERWORK
Prior to your appointment, please complete and submit the intake form sent by Zocdoc upon booking, or complete and return the attached intake form by email. You will receive additional, new patient paperwork via email as well. If this paperwork is not completed prior to your appointment, please arrive at least 15 minutes early to complete the required paperwork in-person.
INSURANCE & PAYMENT
If we do not receive your insurance information at least 24 hours prior to your appointment, we will not be able to verify your benefits until after your appointment. In this case, we will collect a deposit of $100 at the time of service. This will be applied towards any out-of-pocket costs for services, including copays, coinsurances, deductibles, or the full cash rate if we are out-of-network with your insurance provider. Any outstanding balance will be charged to the card on file once benefits are received, and any overpayment can be credited towards a future visit or refunded to the card on file. We are in-network with Medicare and most commercial PPO policies, including but not limited to Aetna, BlueCross BlueShield, Cigna, Humana, Tricare and United Healthcare. We are out-of-network with commercial HMO policies, Medicaid and other government-funded health insurance providers.
