Dr. Trystan Blake, DO, is a board-certified family medicine physician at CommonSpirit Primary Care Salt Lake. He brings a comprehensive and preventative approach to patient care, believing strongly in addressing the whole person rather than just individual symptoms. His care philosophy centers on the idea that proactive health management is key - preventing disease is always easier than managing its complications.
Dr. Blake earned his dual undergraduate degrees in Chemistry and Biology from Southern Utah University in Cedar City before attending Rocky Vista University College of Osteopathic Medicine in Ivins, Utah. He completed his residency at Garnet Health Medical Center in Middletown, N.Y.
Outside of his medical practice, Dr. Blake enjoys a wide range of creative and technical hobbies, including woodworking, CNC machining, construction and design, freeze drying, sewing, 3D printing, sublimation, and hiking. Dr. Blake creates a welcoming environment for all and is committed to providing respectful and sensitive care to transgender, non-binary, and gender-expansive patients.

Dr Bates seemed like he was listening but I do t think he truly cares. And didn’t seem to care the pain has gotten worse and more scary is that now my entire foot is tingly where it was only 3 of my toes. He also ‘prescribed’ a topical medication that is over the counter and is PAINFUL hot pepper gel that burns for hours. After googling today there are other creams but I highly doubt they help with numbness. I have been doing my exercises. Not very happy. And I don’t know what to do. I barely have Medicare and can’t afford multiple multiple doctor visits. I usually don’t write bad reviews. But I think I should have been taken more genuinely by him and not dismissive of the seriousness of tingly to numb toes (now foot sometimes). He also asked me to come back for the same blood tests for my liver and kidneys that were done at the hospital 3 weeks ago

Dr. Trystan is incredibly insightful and guided me on some of my health metrics. He is a great listener, provides feedback on your health scores but more importantly offers to help and expand on any situations you are in.
I do not regularly visit a doctor and naturally get nervous - this is the first doctor's appointment where I felt very happy and lucky that I found Dr. Trystan.

Excellent experience. Dr. Blake was attentive, patient and thorough. As a member of the lgbt community, I've had some bad experiences and not felt welcome with other doctors. I’m 32 years old and had never established care with a primary care doctor before, mostly because I thought doing so was overwhelming.
