Dr. Trystan Blake, DO, is a board-certified family medicine physician at CommonSpirit Primary Care Salt Lake. He brings a comprehensive and preventative approach to patient care, believing strongly in addressing the whole person rather than just individual symptoms. His care philosophy centers on the idea that proactive health management is key - preventing disease is always easier than managing its complications.
Dr. Blake earned his dual undergraduate degrees in Chemistry and Biology from Southern Utah University in Cedar City before attending Rocky Vista University College of Osteopathic Medicine in Ivins, Utah. He completed his residency at Garnet Health Medical Center in Middletown, N.Y.
Outside of his medical practice, Dr. Blake enjoys a wide range of creative and technical hobbies, including woodworking, CNC machining, construction and design, freeze drying, sewing, 3D printing, sublimation, and hiking. Dr. Blake creates a welcoming environment for all and is committed to providing respectful and sensitive care to transgender, non-binary, and gender-expansive patients.

Excellent experience. Dr. Blake was attentive, patient and thorough. As a member of the lgbt community, I've had some bad experiences and not felt welcome with other doctors. Dr. Blake listened to all my concerns and took the time to answer all my questions thoroughly which really built my confidence in the care I'm receiving. The entire staff was friendly, professional, and quick. Would highly recommend.
