* If your insurance plan requires a referral from your Primary Care Doctor (ie: BCBS HMO, UHC Health Select, etc), we will not schedule an appointment without receiving the referral from your primary care provider first.
* Completed New Patient Registration Forms must be submitted to our office via fax or email prior to your appointment. Failure to do so may result in rescheduling your appointment.
* Our office charges a $25 No Show fee.
* Call our office for further assistance.
