Getting to know Dr. Zachary Poliacoff

Dr. Poliacoff completed medical school and residency at the University of South Florida, where he won multiple teaching awards and published several papers on antidepressants. He has worked on several highly specialized services: the James Haley VA's outpatient PTSD program, its Spinal Cord Injury unit as a psychiatric consultant, and its inpatient/outpatient substance use program. He was also the sole psychiatrist at Riverside Recovery, a residential substance use treatment program, and on both Tampa General Hospital's and the James Haley VA's psychiatric consult services for medically complex patients. Dr. Poliacoff currently works at Bellevue's Comprehensive Psychiatric Emergency Program, where he’s a clinical educator for NYU's medical students and psychiatry residents. He is familiar with the latest developments in psychiatric treatment, including the newest pharmaceutical agents and the most up-to-date evidence for a variety of non-pharmaceutical supplements, as well as older, less commonly used but highly effective drugs like MAOIs and TCAs. He has extensive psychotherapeutic training with a focus on substance use and anxiety spectrum disorders.

What should a new client know about working with you?

I aim to provide high quality, personalized, and holistic mental health care with close attention paid to your wants and needs. I see myself as your partner in your mental wellbeing, not just your treatment provider. Together, we will find the right balance of pharmacologic and non-pharmacologic strategies to help you meet your goals, whatever they are. Although I do prescribe controlled substances such as stimulants for ADHD, benzodiazepines for short term management of anxiety or outpatient alcohol withdrawal management, and Z-drugs for insomnia, these are only considered after a thorough evaluation.

What is your typical process for working with clients?

During our initial sessions, I conduct a thorough diagnostic assessment through the "biopsychosocial" lens - I aim to understand the biological, psychological, and social aspects of both who you are and what is causing you to seek treatment. I believe in a collaborative approach, where you can use my expertise to inform your own decisions about what your treatment looks like. For many clients, this includes medication management. I am skilled at helping clients find both pharmaceutical and non-pharmaceutical options. I will advise you on how the potential risks and benefits of suitable options compare, and together we will monitor your progress to ensure your treatment is working. Depending on your needs, our sessions may also include talk therapy, such as cognitive-behavioral therapy (CBT), which helps you find the mental tools that will allow you to realign distorted patterns of thinking with reality to ease anxiety, depression, and distress. For suitable patients I also provide CBT for insomnia and prolonged exposure therapy for specific fears and phobias. Throughout our work together, I emphasize ongoing assessment and adjustment of your treatment plan. Regular follow-up sessions allow us to monitor your progress, address any new or continuing issues, and make necessary adjustments to your care. My ultimate goal is to support you in achieving a higher level of functioning and an improved quality of life.

I am out of network with all insurance plans because insurance reimbursements require higher patient volumes at the expense of individualized attention to be financially viable. Clients are therefore responsible for appointment fees themselves. However, we can provide a 'superbill' which you can then submit to your insurance provider for partial to full reimbursement for our fees. Sliding scale fees are also available to students and healthcare professionals.

I visited Dr. Poliacoff regularly in 2023/2024. He was an amazing partner in navigating the stress and anxiety that was consuming my daily life, caused by sudden family changes.
