Who is a nurse practitioner?

Nurse practitioners (NPs) obtain training in evaluating and diagnosing medical conditions and prescribing and interpreting medical tests. They are also called Advanced Registered Nurse Practitioners (ARNPs) or Advanced Practice Registered Nurses (APRNs). Depending on their specializations, they can provide primary and urgent care services for people of various ages. They often practice in coordination with physicians and also in hospital settings. Nurse practitioners are usually part of teams that provide continuing services to patients with chronic conditions or complex problems.

Educational qualifications of nurse practitioners

Aspiring nurse practitioners must first earn a diploma and a degree in nursing (ADN). Another alternative is to obtain a Bachelor of Science degree in nursing (BSN). An additional degree, such as a Master of Nursing Science (MSN) or Doctor of Nursing Practice (DNP), helps nurse practitioners gain advanced knowledge. They must clear a licensure examination before practicing and maintain the validity of the license through continued education. NPs can also pursue specialization and obtain a certification in that particular area.

History of nursing practice as a profession

Here is a brief overview of the historical timeline of nursing practice as a profession:

  • 1960s: In 1965, Dr. Loretta Ford and Dr. Henry Silver developed the first nurse practitioner (NP) program at the University of Colorado. After two years, in 1967, Boston College initiated one of the first master's programs for NPs named the Boston-based Bunker Hill/Massachusetts General Nurse Practitioner Program. It officially began in 1968 under the direction of a nurse and physician team.
  • 1970s: The University of Washington began the first ever family NP program, PRIMEX, in 1971. Till 1973, there were over 65 NP programs established in the USA, and the National Association of Pediatric Nurse Practitioners came into existence. In 1974, The American Nurses Association created the Council of Primary Care Nurse Practitioners. In the same year, The Burlington Randomized Trial Study found that NPs make appropriate referrals when medical intervention is necessary. In 1975, The University of Colorado offered its first continuing education symposium for NPs. The Association of Faculties and Pediatric Nurse Practitioners was established in 1978. Until 1979, there were around 15,000 NPs practicing in the USA.
  • 1980s: In 1980, aspiring students were allowed to enroll in more than 200 NP programs. In the same year, Nurse Practitioner Associates for Continuing Education was also established. Guidelines for Family Nurse Practitioner Curricular Planning were published after five years of development at the University of New Mexico. In 1983, the number of nurse practitioners practicing in the USA reached about 24,000. Then, in 1985, The American Academy of Nurse Practitioners (AANP) came into existence. It initiated the development of a national NP database and had 100 members at the end of its first year. Until 1989, 90% of NP programs were either master’s degree programs or post-master's degree programs. The year also saw the publication of the Journal of the American Association of Nurse Practitioners, and the AANP moved its Massachusetts headquarters to Texas.
  • 1990s: In 1992, the AANP began actively working with nursing associations like the Royal College of Nursing UK. It aimed to develop the role of NPs internationally. In 1993, the AANP formed the American Academy of Nurse Practitioners Certification Program. Until 1999, there were approximately 68,300 NPs practicing in the USA.
  • 2000s: The number of practicing nurse practitioners in the USA increased to approximately 82,000 in 2001, 97,000 in 2003, 106,000 in 2004, 120,000 in 2007, and 130,000 in 2009. According to the AANP’s 2022 estimates, there are more than 355,000 NPs practicing in the USA.

What are the different kinds of nurse practitioners?

Depending on their specialization, NPs are highly trained healthcare professionals who care for specific patient populations. You must search for the type of nurse practitioner near you who fits your requirements. Here are the main types of nurse practitioners:

  • Family nurse practitioner (FNP): FNPs are primary care professionals offering comprehensive healthcare services to patients of all ages. They develop long-term relationships with patients and often serve as primary care providers.
  • Adult nurse practitioner: Adult NPs provide primary care services to adult patients, including examinations, education, prescriptions, and treatments.
  • Pediatric nurse practitioner (PNP): PNPs specialize in delivering primary care to children from infancy to teenage. They conduct well-child check-ups, administer immunizations, and diagnose and treat acute and chronic conditions.
  • Neonatal nurse practitioner: Neonatal NPs care for sick and premature babies, working in labor and delivery units and neonatal intensive care units. They diagnose illnesses and provide treatment, sometimes assisting in deliveries.
  • Geriatric nurse practitioner: Geriatric NPs work with older adults and may specialize in specific groups like nursing home residents or individuals with chronic conditions like diabetes. They offer primary care and patient education.
  • Women's health nurse practitioner: These NPs focus on women's health concerns, providing comprehensive reproductive and gynecological care.
  • Acute care nurse practitioner: Acute care NPs treat adults in urgent care centers or emergency rooms, addressing sudden and severe injuries or illnesses and post-surgery care.
  • Occupational health nurse practitioner (OHNP): OHNPs treat workplace injuries and educate employees about health and safety.
  • Psychiatric nurse practitioner: Psychiatric NPs specialize in mental health care, treating specific mental health disorders and substance use issues.
  • Emergency nurse practitioner: Emergency NPs work in emergency departments, providing urgent care, making critical treatment decisions, and monitoring patients' conditions.
  • Adult-gerontology nurse practitioner: Adult-gerontology NPs offer healthcare for adults of all ages, managing diseases and chronic health conditions in this diverse population.
  • Orthopedic nurse practitioner: Orthopedic NPs focus on musculoskeletal issues, including diseases and injuries affecting muscles, bones, joints, and connective tissue.
  • Aesthetic nurse practitioner: Aesthetic NPs care for patients undergoing cosmetic procedures, offering counseling, performing procedures, and assisting with recovery.
  • Oncology nurse practitioner: Oncology NPs provide comprehensive care to patients with cancer, collaborating with oncologists to develop treatment plans and manage their overall care.

Each NP specialty requires specific education, exams, and licensure, enabling these skilled professionals to deliver specialized and high-quality care to their respective patient populations.

Difference between nurse practitioner and registered nurse

Nurse practitioners and registered nurses (RNs) share a common objective: delivering healthcare services to promote optimal patient outcomes. Nevertheless, their roles and responsibilities differ significantly. RNs frequently serve as patients' initial point of contact, offering fundamental primary care. In contrast, NPs assume more advanced or specialized duties in patient care.

Registered nurses constitute the largest group of nursing professionals, collaborating with various healthcare providers, such as physicians, evaluating patients, and executing treatment plans. Their expertise is indispensable across various medical domains, as they are trained to deliver many fundamental healthcare services within medical facilities. RN duties encompass:

  • Documenting symptoms and updating patient records
  • Vigilantly monitoring patients and documenting observations
  • Administering treatments as directed by the attending physician or nurse practitioner
  • Assisting physicians or nurse practitioners in conducting diagnostic tests
  • Advising patients' families on managing healthcare challenges.

Registered nurses can specialize in diverse areas such as addiction nursing, cardiovascular nursing, critical care nursing, or public health nursing.

Who is an advanced practice registered nurse?

An advanced practice registered nurse, abbreviated as APRN, operates at an elevated level within the nursing profession and delivers specialized care to their patients. Advanced practice nursing encompasses several specialized branches:

  • Nurse midwives offer advanced services to women, including gynecological examinations, family planning assistance, and prenatal or postnatal care.
  • Nurse anesthetists administer different types of anesthesia to patients before, during, and following surgical, therapeutic, diagnostic, or obstetrical procedures.
  • Clinical nurse specialists diagnose, treat, and manage ongoing healthcare for diverse patients. Additionally, they act as advocates for change and serve as valuable sources of information for registered nurses within their organizations.
  • Nurse practitioners near you deliver primary and specialized care to patients. They often receive training to provide advanced nursing services tailored to specific patient populations, such as children or individuals with mental health concerns.

Where are nurse practitioners required?

Nurse practitioners are essential to the community as healthcare providers, educators, researchers, and mentors. They can provide palliative care and pain management in the home healthcare setting. They can assist patients who are not sick enough for hospitalization but still require care, such as recuperating post-surgery or an extended illness. At the hospital, nurse practitioners care for in-patients and act as conduits of communication and patient education.

Nurse practitioners also help treat patients in outpatient urgent care settings, including those with injuries and acute health issues. In private practice, they see patients of all ages for various ailments. In some states, they work under a physician; in others, they could work as independent practitioners. Nurse practitioners also serve as treatment team members in skilled nursing facilities (SNF).

How are nurse practitioners different from doctors?

Doctors and nurse practitioners both provide direct medical care to their patients. While all doctors can prescribe medications to patients, in some states, nurse practitioners can write prescriptions only under a physician's or doctor's supervision. The education paths to these two careers differ. Becoming a nurse practitioner takes six to eight years, while becoming a fully licensed physician takes 10 to 14 years. As a result, nurse practitioners can begin their careers much earlier than doctors.

The scope of functioning for nurse practitioners will differ from state to state. A doctor's license has more authority, privileges, and responsibilities than other healthcare providers. Both types of professionals can choose to specialize in their areas of interest, although there is a slight difference in the specializations available. Doctors usually work in private or group practices, hospitals, and clinics. Some pick academia, and others work for the government. Often, nurse practitioners work in doctors' offices, outpatient care centers, and educational services.
