Who is a primary care doctor?

Primary care doctors are responsible for providing comprehensive healthcare services to help their patients stay healthy. They act as the direct contact for their patients in case of any health concerns. Their service group includes all ages, genders, organs, and diseases. Their day-to-day responsibilities include:

  • disease prevention and screenings
  • diagnosis and treatment of everyday illnesses and injuries
  • managing chronic issues
  • health education
  • lifestyle and diet counseling
  • coordinating care with the healthcare team

Different types of primary care physicians cater to various age groups. Family physicians provide care for individuals and families throughout all age groups. Internists provide services to only adults over 18 years old. They typically work with older patients with multiple chronic illnesses. Pediatricians offer primary care services to children and adolescents from birth until 18-21 years old. Obstetricians and gynecologists (OB-GYNs) are experts in women's health, and geriatricians care for seniors aged 65 and over.

What is the education and training of primary care doctors?

Primary care doctors receive extensive education and training for at least 11 years. They start by acquiring a four-year undergraduate degree in pre-medical courses such as biology and chemistry. Then, they enroll in a medical school where they train for another four years and graduate with a DO (doctor of osteopathic medicine) degree or an MD (doctor of medicine) degree.

After that, they complete a three-year residency program in family medicine, internal medicine, pediatrics, gynecology, and obstetrics. They can also take exams to get certified by the American Osteopathic Board of Family Physicians and the American Board of Family Medicine. Post-residency, primary care physicians can start practicing with a state license or extend their training with one to two years of fellowships.

Primary care doctors and their subspecialties

Primary care doctors play an essential role as they are the first point of contact for patients who need medical assistance in the healthcare system. They are responsible for treating and managing diverse healthcare needs, encompassing preventive care, routine physical examinations, screenings, accurate diagnosis, and management of chronic conditions.

Moreover, primary care doctors have various subspecialties suited to different patients' needs. Some of the common ones include:

  • Pediatricians: They specialize in diagnosing and treating medical conditions and illnesses in babies, children, and adolescents, thus playing a vital role in helping them cross developmental milestones and ensure their healthy future. Pediatricians also create a welcoming and safe environment for the children during their medical visits for their care.
  • Internists: These physicians, also known as internal medicine doctors, provide exclusive care to adults. They can diagnose and treat simple and complex medical conditions affecting people from early adulthood through their senior years. Internists prioritize preventive care and pay special attention to conditions such as diabetes and high blood pressure.
  • Family medicine doctors: Unlike pediatricians and internists, family doctors treat patients of all ages. Hence, they provide comprehensive healthcare services for the entire family across multiple generations. They take time to familiarize themselves with the family's medical history to offer personalized care.
  • Obstetricians and gynecologists (OB-GYNs): They focus on the female reproductive system, including the breasts, pelvis, vagina, vulva, cervix, uterus, fallopian tubes, and the ovaries. OB-GYNs treat various medical conditions, including screening for early signs of breast cancer and sexually transmitted infections. They also provide options for birth control, family planning, care during pregnancy, and support throughout menopause.
  • Geriatricians: These doctors are significant in treating and caring for seniors over 60 and above. They diagnose and treat a range of conditions and diseases common in older adults, including issues related to falls, memory problems, medication management, and acute and chronic conditions.

What conditions does a primary care doctor treat?

Primary care doctors have a vast knowledge base about various health conditions affecting people from birth until their senior years. Below listed are some of the conditions that they treat

  • Common acute illnesses include coughs and colds, sore throats, chickenpox, sinus infections, flu, ear infections, urinary tract infections (UTIs), insect bites, and bronchitis.
  • Common injuries include wounds, cuts, scratches, sprains and strains, pulled muscles, whiplash, and minor burns.
  • Skin conditions such as mild rashes, sunburns, and dermatitis.
  • Gastrointestinal conditions include diarrhea, gastrointestinal reflux disease (GERD), gallstones, gastritis, irritable bowel syndrome (IBS), and ulcers.
  • Breathing ailments such as chronic obstructive pulmonary disorder (COPD) and asthma.
  • Endocrine conditions include diabetes (Type 1 and Type 2), hypothyroidism, and hyperthyroidism.
  • Cardiac diseases like high blood pressure, high cholesterol, and coronary artery disease.
  • Other chronic illnesses include osteoporosis, cancer, stroke, Alzheimer's disease, depression, and anxiety.

Difference between primary, secondary, and tertiary care

Primary care is your initial stop for most medical concerns, including illness, injury, chronic conditions, and specialist referrals. You may visit a doctor, nurse practitioner, or physician assistant for regular checkups, screenings, and wellness visits. Studies have shown that primary care providers improve access to healthcare, enhance health outcomes, and reduce hospitalizations and emergency department visits.

When a primary care provider sends you to a specialist, it is called secondary care. This means that your healthcare provider has transferred your care to someone with more specific expertise in the health issue you are experiencing. Your primary care provider stays in contact with the specialist.

Specialists focus on a specific body system or a particular disease or condition. Examples of specialists include:

  • Cardiologists who focus on the heart and blood vessels
  • Endocrinologists who focus on hormone systems, including diseases like diabetes and thyroid disease
  • Oncologists who specialize in treating cancers, and many focus on a specific type of cancer

If you are in the hospital and need specialized care, your doctor might refer you to a tertiary care center. These centers have advanced equipment and expertise to treat complex health conditions.

Some examples of treatments and procedures offered at these centers include plastic surgery, neurosurgery, organ transplants, head and neck oncology, trauma surgery, high-dose chemotherapy for cancer, dialysis, coronary artery bypass graft (CABG), burn treatment, etc.

Local hospitals may need to be equipped to provide this level of care so that you might be transferred to a tertiary care center.

What can you expect during an appointment with a primary care doctor near you?

Before visiting your primary care doctor, make a list of health concerns, symptoms, queries, and fitness goals you wish to achieve. It will also help if you carry your past and current medical records and medications list.

If it is your first visit, the doctor will try to get as many details as possible from you. You can expect questions regarding your medical history, family history, past and current medications, supplements, and lifestyle and diet habits. They may also conduct a head-to-toe physical exam, screening tests, and order lab tests to assess your overall health status. Based on your results, the doctor will recommend you repeat these preventive tests annually or every few years for early detection and prevention of future problems.

They will guide you on healthy lifestyle and diet choices to improve or maintain your health and prevent diseases. If you have any specific health problems, you can discuss your symptoms with them. Your doctor can diagnose the condition, prescribe medications and other treatments, and arrange follow-up visits. They can also refer you to specialists if required. Primary care providers are essential in chronic disease management by facilitating regular screenings, checkups, treatment monitoring, coordinating care, and patient education.

What happens during your annual physical exam?

Your annual check-up with a primary care provider typically involves two important components: a discussion about your current health and a detailed physical examination. This routine visit allows you and your healthcare provider to review your overall well-being, address any concerns, and identify potential health issues early.

Each appointment can vary slightly based on individual needs and the provider's approach. To start, your doctor will review your recent health history. This includes asking about any new symptoms, changes in your family's medical background, or general health concerns you might have noticed. For instance, individuals who menstruate may be asked if there have been any irregularities in their cycle. Your doctor might also inquire about lifestyle factors such as diet, exercise, alcohol consumption, and tobacco use.

The next part is the physical examination. This comprehensive checkup allows your provider to detect signs of issues that might not yet cause noticeable symptoms. They may check your pulse, listen to your heart and lungs, examine your ears and mouth, examine your skin, and feel your lymph nodes. If necessary, they may also check for swelling in your limbs and perform a breast exam.

Additional routine steps often include measuring your height and weight to figure out your BMI (Body Mass Index). This number can indicate potential risks for conditions like heart disease, cancer, or stroke. You can always express discomfort if you prefer not to hear your weight read aloud. Vital indicators, like blood pressure and pulse, will also be taken. High blood pressure is a known risk factor for heart attacks, kidney damage, and other serious conditions.

Depending on your age and anatomy, your provider might perform internal exams. For example, pelvic exams or Pap smears for individuals with vaginas or prostate and testicular exams for those with prostates. Finally, blood tests may be recommended to assess cholesterol, glucose, or organ function, aiding in the early detection of chronic illnesses.
