About Dr. David Chadwick
David Chadwick, DO was born in California and raised in the small town of Marion, WI. Dr. Chadwick started his career in healthcare working as a nursing assistant in an assisted living facility while completing his Bachelor’s Degree. His positive experience as a nursing assistant led him to pursue a career in medicine. After graduating from Maranatha Baptist University in Wisconsin, he then moved to Glendale, AZ where he completed his Master of Arts in Biomedical Sciences at Midwestern University. Dr. Chadwick attended the same university for medical school and completed his doctor of osteopathic medicine degree from Arizona College of Osteopathic Medicine.
Dr. Chadwick trained in residency at St. Joseph’s Hospital and Medical Center in Phoenix, AZ where he gained diverse clinical experience including hospital medicine, chronic disease management, emergency medicine, geriatrics, and pediatrics. Dr. Chadwick is committed to improving the lives of his patients through quality, patient-centered health care. He enjoys seeing patients of all ages, from children to geriatric.
Outside of his job in healthcare, Dr. Chadwick enjoys reading, hiking, trying different restaurants, traveling, concerts, and sports.

Dr. Chadwick and his assistant did a thorough job of recording my past medical procedures and blood testing done the previous week. This was an annual physical exam so I felt they did well for our first meeting. I have a follow up in 3 months to check a medications efficacy. I would recommend this Doctor.

I was so nervous! I will not tell you how long it had been since I had gone a doctor's appointment of my own volition, but it's been years. Being a lapse patient makes me even more anxious, I did not need to worry.
I appreciate all my care with this office, from a call I made to the front desk before my appointment, to the Physician's Assistant who put me at ease, to Dr. Chadwick's ability to get a good understanding of my needs quickly and come up with next steps, to an on-site phlebotomist. My care was beyond my expectations.

The office is not very appealing. It looks old and not put together, there was a Christmas tree still up in the corner. Dr. Chadwick was nice and seemed to listen. My biggest problem is he didn't finish the appointment. He never came back into my room to discuss plan of care and orders, all of a sudden the MA comes back in and hands me my orders. Due to the lack of communication one of my diagnostic test was not ordered correctly and this delayed care. There was no shared decision making, no clear plan and no follow up. I will not be going back.

i overheard the girl at the front desk stating to a coworker that dr chadwick had to meet a quota of 22 patients per day right before i was seen. appointment felt rushed and was over in 5 minutes. i did not receive a referral for the problem that i had scheduled the appointment for.

Aside from a little extra wait time which they apologized for I was very pleased with the visit. Nurse practitioner was very helpful and pleasant to be with and listened to my needs. Dr Chadwick was superb. He listened to me as well, made some changes to my meds and made me feel like he cared.

Great initial visit with office. Couldn’t have gone any easier, from appointment to registration to check in and visit. Dr Chadwick was courteous and attentive, office check-in reception was super friendly. Everyone was fantastic including the lab folks upstairs.

The ZocDoc experience was quick and simple. I was able to get in to see the provider in less than a week. Selecting and confirming the appointment was easy. At the actual appointment the front office staff was surprisingly patient and friendly despite multiple personnel being out sick and one lady handling it all on her own. Dr. Chadwick was prompt, courteous, and knowledgeable. He listened to my concerns. My only issue is that I had some labs drawn and the results may have been off due to not long enough of a fast. I've sent a message through the provider website and I'm waiting for the doctor to call me back to discuss the results and schedule a planned re-test to ensure they are accurate, though it's been longer than 48 hours and the provider portal states that all messages will be replied to in 48 hours or less. Otherwise, would recommend!
