Nathan Fowler APRN has been with C & C Health Care Services since 1991. Nathan Graduated with his MSN in 2017. Nathan not only provides Primary Care he also works with Mental Health patients.
The mission of C & C Health Care Services is to enhance the well-being of the community by providing comprehensive health care services including prevention, education, primary care, behavioral health, addiction services, & wound care services with a special emphasis on the military, medically underserved, and low-income populations. Our goal is to provide a “medical home” for the underserved by significantly expanding access to primary care, behavioral health, disease management, prevention, and educational services.
