“I was able to book an appointment on line and was seen on time. I was very happy with the PA that examined me. The only problem I had was the PA wrote a conflicting statement to the notes in my chart. I said I never smoked and that was put in my file. The PA also stated that I quit smoking at age 29, which an untrue statement. I don’t know where that statement came from. I hope it can be removed from my file. I would appreciate any assistance in removing the statement that I quit smoking at age 29. I have NEVER SMOKED!”
