About Dr. Sophia Kostelanetz

Dr. Kostelanetz is a board-certified Internal Medicine and Pediatrics physician who provides comprehensive care for children, adolescents, and adults. Her clinical focus includes preventive services and the management of chronic conditions such as diabetes, hypertension, asthma, and cardiovascular disease. She is dedicated to patient-centered care, emphasizing attentive listening, shared decision-making, and long-term wellness. She earned her undergraduate degree, MD, and MPH from Tufts University and worked with Partners In Health on health systems strengthening and quality improvement initiatives in Haiti and West Africa. She completed her Internal Medicine–Pediatrics residency at Vanderbilt University Medical Center, followed by an AHRQ-funded postdoctoral fellowship in population health, quality improvement, and implementation science. Dr. Kostelanetz has held leadership roles advancing population health and health equity at local and national levels. As Health Equity Lead at One Brooklyn Health, she helped expand kidney transplant access, lung cancer screening, and social determinants of health screening. She has also worked with the Tennessee Department of Health and partnered with the NYC Department of Health to improve chronic disease care, access, and medical training. Her research focuses on addressing structural drivers of disparities and improving primary and chronic disease care. She currently serves as Associate Director of the Division of Internal Medicine and Medical Director of the Nostrand Clinic at Maimonides Health, where she is committed to delivering compassionate, high-quality care and improving health outcomes for diverse communities.
