Doriel Jacov, LCSW, JD

Psychotherapist
280 Madison Ave, New York, NY
I'm a licensed psychotherapist (LCSW) offering psychodynamic, trauma-informed therapy for adults and couples in New York City and across New York State via telehealth. Many of the people I work with have been managing for a long time. They're functional, often high-achieving, and privately struggling with things that haven't resolved no matter how much insight or effort they've brought to them. Patterns in relationships that repeat. A sense of not being quite enough, or of feeling disconnected from themselves and others. The particular exhaustion of complex trauma or chronic emotional neglect that was never named as such. Grief, burnout, or the disorientation of a major life transition that has shaken something loose. My approach is depth-oriented and relational. Rather than focusing primarily on symptom management or coping skills, we work to understand the emotional and relational patterns shaping how you experience yourself and others — and how those patterns developed. This often means exploring early experiences, including trauma, and paying attention to what unfolds between us in the room as a window into those patterns. Before becoming a therapist, I practiced corporate law after earning my JD from Columbia Law School. That background gives me firsthand understanding of high-pressure professional environments and the particular kind of burnout that comes from years of putting your interior life aside. I offer a free consultation to see whether working together feels like a good fit.

They're functional, often high-achieving, and privately struggling with things that haven't resolved no matter how much insight or effort they've brought to them. Patterns in relationships that repeat. A sense of not being quite enough, or of feeling disconnected from themselves and others. This often means exploring early experiences, including trauma, and paying attention to what unfolds between us in the room as a window into those patterns. Before becoming a therapist, I practiced corporate law after earning my JD from Columbia Law School. What should a new client know about working with you?

My approach will feel different from more structured, skills-based therapy. There are no worksheets, no homework assignments, and no fixed agenda. Sessions are open-ended and follow what feels most alive or most pressing for you. This kind of work asks something of you: a willingness to explore, to sit with uncertainty, and to bring the full texture of your experience into the room — including things that feel difficult or shameful to say out loud. That's not always comfortable, but it's often where the most meaningful work happens. The early sessions are as much about building trust as they are about exploring content. There's no pressure to disclose everything at once or to have it figured out before we begin. We start wherever you are. I want new clients to know that the therapy relationship itself is central to how I work. I pay close attention to what unfolds between us — not just what you bring in from outside, but what happens in the room — because familiar patterns often emerge there, and working with them in real time is often where the most durable change happens. This approach works best for people who are curious about themselves and open to exploring rather than looking for a structured program. If you're unsure whether it's the right fit, the consultation is a good place to figure that out.

What is your typical process for working with clients?

We begin with a free consultation, usually 15 to 20 minutes, where you can share what's bringing you to therapy, ask questions about my approach, and get a sense of whether working together feels like a good fit. There's no obligation and no preparation required. If we decide to move forward, the first few sessions function as a mutual assessment. I'll ask about your history, your current experience, and what you're hoping to understand or change. You'll get a clearer sense of how I work and whether the relationship feels right. This early period matters — the therapeutic relationship is central to the work, and it deserves to be built carefully. Ongoing sessions are 45 minutes and typically weekly. Sessions don't follow a fixed agenda. We explore what's present for you, what's been activated during the week, and what patterns keep surfacing. I pay close attention to what unfolds both within you and between us, because familiar dynamics often emerge in the therapy relationship and working with them in real time is often where the most durable change happens. Progress in this kind of work is rarely linear. There are periods of significant movement and periods that feel slower. I check in regularly about how the work is feeling and welcome direct feedback, including when something isn't working. I offer a free initial consultation before we begin, so you have a chance to get a feel for how I work and ask any questions you have.
