Getting to know Dr. Thomas Marchevsky
I'm a licensed psychoanalyst and clinical psychologist with more years of experience in helping adults understand themselves and change the patterns that keep them stuck. My work is rooted in psychoanalytic, depth-oriented psychotherapy, which means I'm not interested in quick fixes. I want to help you understand why you feel and act the way you do, and what it would take for something to genuinely shift.
I work with people dealing with anxiety, depression, relationship difficulties, grief, major life transitions, and the kind of recurring patterns that seem to repeat no matter how hard you try to break them. Many of the people I see have been to therapy before and are looking for something deeper than coping strategies: a space to speak without inhibitions about life to someone who will take them seriously.
Alongside my clinical practice, I serve as Editor-in-Chief of a major psychoanalytic journal and remain actively engaged in the field's scholarship. That grounding informs how I work, but in session what matters is you: your history, your questions, and what you're trying to make sense of.

What should a new client know about working with you?
My approach is collaborative and honest. I won't hand you a worksheet or a list of techniques to manage your symptoms. Instead, I'll work with you to understand what's actually driving them, the patterns, beliefs, and experiences underneath what brought you in. I practice psychoanalytic psychotherapy: in plain terms, that means I believe much of what shapes us operates outside our immediate awareness. A lot of the work is about bringing that into view: noticing that which repeats, what gets avoided, and what your symptoms are telling you. This takes curiosity and work , but it's also where lasting change comes from.
You can expect me to listen closely and to be direct with you. I'm not the kind of therapist who only nods and reflects; when I notice something, I'll say so. At the same time, this is your space, and you set the pace. There's no judgment here, and nothing you bring is too small, too strange, or too much.
This way of working tends to fit people who feel stuck, who keep ending up in the same situations, or who sense that something deeper is going on than a single problem they can name. If you want not just to feel better but to understand yourself, we'll likely work well together.

Sessions are 50 minutes and conducted by secure video. Most people meet with me weekly, especially early on, since consistency is what gives the work momentum. We'll decide together what rhythm makes sense for you.
In the first few sessions, I'll want to get to know you — what brought you in now, your history, the people and patterns that have shaped you, and what you hope will be different. There's no rush to define everything immediately; understanding deepens over time.
From there, sessions are largely open conversation. Rather than following a fixed agenda, I invite you to speak freely about whatever is on your mind, and we follow where that leads. I pay close attention to the threads running through what you share — recurring themes, the things that stir strong feeling, dreams, and the patterns that show up in your relationships, including the one we build together. Often the most useful insight arrives not from a technique but from noticing something neither of us expected.
This is talk therapy in the fullest sense. As a psychologist I don't prescribe medication, but I'm glad to coordinate with a psychiatrist if that's something you're considering. My aim is to give you a steady, thoughtful space to speak, make sense of your experience, and gradually free yourself from the patterns that have been holding you back.
