Zachary is dedicated solely to supporting individuals preparing for bariatric surgery who require a comprehensive pre-surgical psychological evaluation.
Mr. Wohlberg holds a master’s degree in Mental Health Counseling from Bellevue University. Mr. Wolberg brings a structured, evidence-based, and compassionate approach to each evaluation, ensuring alignment with bariatric surgical program requirements while maintaining a patient-centered experience.
At this time, Mr. Wohlberg provides bariatric psychological assessments only and does not offer ongoing psychotherapy or treatment for other mental health conditions through this practice.
Mr. Wohlberg is currently paneled with CIGNA only. Clients with other commercial insurance plans may choose to proceed using out-of-network benefits. If you elect to book using out-of-network coverage, the practice can provide a superbill upon request for submission to your insurance carrier for potential reimbursement. Please note that reimbursement is determined solely by your insurance plan, and the provider and practice are not responsible for coverage decisions or reimbursement outcomes.

What should a new client know about working with you?
I am currently accepting all new bariatric clients seeking pre-surgical psychological assessments to support readiness and preparedness for bariatric surgery via Telehealth. At this time the practice is focused exclusively on bariatric psychological evaluations, allowing me to provide a thorough, specialized, and supportive experience throughout the assessment process.
We are currently paneled with Cigna only, and we also work with clients who have other insurance plans under out-of-network benefits. This means we are not directly contracted with most insurance companies; however, if your plan includes out-of-network coverage, a portion of your evaluation cost may be reimbursed. You may wonder why we do not accept additional insurance plans. The answer is simple: high-quality care comes first. Our evaluations are comprehensive, evidence-based, and patient-centered, exploring the full scope of your emotional wellness and readiness. We don’t just determine whether you are a “safe” candidate—we help you feel supported and prepared for the lifestyle changes ahead.
Once your intake forms are finalized, you will meet directly with a licensed clinician experienced in bariatric psychological evaluations. My approach is professional, compassionate, and aligned with surgical program requirements, while also ensuring you feel heard and supported during this important step in your journey. Please note: you are responsible for any late cancellation or no-show fees in accordance with practice policy.
What is your typical process for working with clients?
My work with bariatric clients is grounded in a comprehensive, compassionate, and evidence-based approach to pre-surgical psychological assessment. I understand that bariatric surgery is not simply a medical procedure—it is a life-changing transition that impacts emotional health, relationships, identity, and daily functioning.
As part of the evaluation process, you will be asked to complete a structured psychological questionnaire that is aligned with your bariatric assessment. This allows me to gain a well-rounded understanding of your emotional functioning, coping strategies, and readiness for surgery, and helps ensure that your evaluation is both thorough and clinically sound. Evaluations may be completed virtually via secure Telehealth, depending on your preference. Telehealth appointments are available and commonly used, offering flexibility and convenience while maintaining the same level of care, professionalism, and clinical integrity as in-person visits. During the assessment, I take time to explore your emotional readiness, relationship with food, stress management, support systems, and ability to adapt to long-term lifestyle changes required after surgery.
My goal is not to “pass or fail” you, but to ensure that you feel informed, supported, and mentally prepared for the journey ahead. I work collaboratively and without judgment, creating a space where you can speak openly and honestly. All evaluations are conducted in alignment with surgical program requirements, while maintaining a patient-centered, therapeutic perspective. Above all, I believe bariatric psychological evaluations should feel thorough, ethical, and empowering—not rushed or transactional. My role is to help you move forward with clarity, confidence, and emotional preparedness as you take this important step toward improved health.
