Who is a chiropractor?

A chiropractor, Doctor of Chiropractic (D.C.), or chiropractic physician employs a hands-on approach to diagnosing, treating, and managing various health conditions. They possess comprehensive diagnostic skills and training that allow them to conduct patient examinations, make diagnoses, and administer treatments. Chiropractors also recommend rehabilitative exercises, therapeutic practices, and guidance on nutrition and lifestyle choices.

These experts utilize various methods to determine the appropriateness of chiropractic treatment, such as clinical examinations, laboratory tests, imaging techniques, and other medical interventions. If chiropractic care is not suitable for a patient's specific condition or if co-management with other healthcare providers is necessary, chiropractors readily refer their patients to the appropriate healthcare professionals.

One of chiropractors' primary focuses is treating musculoskeletal problems like neck and back pain, headaches, and joint discomfort in the arms or legs. By utilizing manual techniques and adjustments, chiropractors aim to help reduce pain, improve mobility, and enhance overall well-being.

What are the educational qualifications of a chiropractor?

To become a chiropractor in the United States, individuals must have specific educational qualifications. Becoming a chiropractor includes the following:

  • Earning a Doctor of Chiropractic (D.C.) degree
  • Meeting the requirement of passing the National Board of Chiropractic Examiners' test with success
  • Obtaining a state license

In addition, several states mandate chiropractors to pass an exam on state-specific laws, and all states require chiropractors to engage in continuing education to maintain their licensure. Aspiring chiropractors typically complete at least three years of undergraduate education to enroll in a D.C. program in the United States. The Council on Chiropractic Education which is recognized by the U.S. Secretary of Education, grants accreditation to universities that offer D.C. degrees.

Chiropractic education encompasses a combination of academic coursework and supervised clinical experiences. Candidates develop and practice various skills, including adjustment techniques, spinal assessments, and diagnostic procedures. Coursework covers a range of subjects, including general sciences such as anatomy and physiology. Some chiropractors may pursue specialization in specific fields, such as orthopedics or pediatrics.

The range of chiropractic services that chiropractors are authorized to offer varies across different states in the United States. While health insurance plans generally cover chiropractic services, the coverage is typically partial and not comprehensive. Aspiring chiropractors must know their practice-specific requirements and regulations and stay updated on continuing education requirements for ongoing professional development and compliance with licensure obligations.

Types of chiropractic practitioners

Though the field of chiropractic has evolved, some chiropractors continue to believe in unscientific concepts. Chiropractors who adhere to theories most other professionals have abandoned are known as "straights." Before scheduling an appointment with a chiropractor near you, it is a good idea to learn whether they follow a modern or traditional approach. This allows a person to make an informed decision about the type of treatment they will receive.

People with musculoskeletal issues may also want to consider treatment options such as physical therapy, exercise therapy, and other medical procedures. It is usually a good idea to consult a doctor before choosing a treatment plan.

What’s the difference between a chiropractor and an osteopath?

Chiropractors and osteopaths both focus on improving the function and structure of the body by working with bones, connective tissues, and muscles. However, there are some differences in their training and approach. Osteopaths typically complete a four-year course to earn a BSc (BOst) degree. At the same time, chiropractors undergo 4-5 years of training, which includes a BSc in human sciences and chiropractic, followed by a year of postgraduate training to earn their chiropractic degree.

Both professionals use a technique called the “high-velocity thrust,” a short, sharp motion applied to the spine. Chiropractors generally perform this with their hands, whereas osteopaths often use movements involving the patient’s arms and legs. Both fields share additional techniques.

Chiropractors could use X-rays as a diagnostic tool, whereas osteopaths generally use them to rule out other potential issues.

Treatment frequency also varies: a chiropractor may recommend a series of six sessions, initially close together and then spaced out weekly. Osteopaths, however, tend to offer appointments on an “as-needed” basis.

What are chiropractic adjustments?

Chiropractic adjustments are non-invasive procedures in which chiropractors use manual force to realign or manipulate joints. Spinal manipulation is the most common and well-accepted adjustment performed by a chiropractor.

During an adjustment, the chiropractor asks the patient to lie in a particular position. They will then apply a controlled or sudden force to push the joint beyond its range of motion. You may also hear popping sounds. These are sounds of gasses like carbon dioxide, oxygen, or nitrogen released from your joints as they realign.

After a chiropractic adjustment, you may feel mild soreness or slight discomfort, which should go away in a day or two. If you experience any concerns or need additional guidance, it's advisable to consult a chiropractor near you. Your chiropractor may suggest exercises for you to do at home to help your condition, like hot and cold compresses. Besides therapeutic care, a chiropractor advises on a patient's diet, nutrition, weight control, ergonomics for proper posture, and stress management.

What other services and treatments do chiropractors provide?

Besides performing adjustments to the joints and spine, chiropractors also provide other forms of therapy when required, such as:

1. Transcutaneous Electrical Nerve Stimulators (TENS): Transcutaneous Electrical Nerve Stimulators (TENS) are a form of electrical stimulation in which low-voltage electric currents are delivered through electrodes to alleviate chronic pain. This electric current stimulates the nerve cells and blocks pain signals enabling release of endorphins, the body's natural pain relievers. TENS therapy treats chronic pain in the pelvis, lower back, fibromyalgia, and tendonitis.

2. Cold laser therapy: Also known as low-level laser therapy, it is a treatment that uses low light to provide pain relief. During this procedure, your provider will hold a flashlight-like handheld device over the pain area for several minutes. Photons of light from the laser penetrate through the layers of skin and reduce swelling, back pain, and inflammation. This procedure also improves circulation.

3. Hydrotherapy: Hydrotherapy uses water's versatile properties to relieve pain symptoms. Some techniques include hot/cold water compressions, whirlpools, saunas, and whole-body immersion tanks. Studies suggest hydrotherapy reduces pain and relaxes muscles, improving blood flow. It may also help to reduce pain associated with arthritis.

4. Ergonomics: Chiropractors may be able to identify ergonomic issues that could cause pain in your neck, back, or shoulders. They can advise on how to make your workplace more comfortable by changing the position of your chair or desk and suggest specific tools like laptop stands, ergonomic keyboards, and kneeling chairs to avoid physical stress on your body.

What are the benefits of chiropractic care?

There are several reasons one might choose chiropractic care over conventional treatments to relieve muscle, tendon, spine, and joint pain, such as:

  • Low dependency on opioid pain medications:

Medication helps reduce pain successfully. However, alternatives like chiropractic adjustments may provide the same satisfaction while keeping your system free of painkillers. You can also manage your pain with a balanced combination of medications and chiropractic care.

  • An affordable way to treat chronic back pain:

Research shows that chiropractic care is more affordable than surgery or medication for lower back pain. Adjustments and exercises by a chiropractor are highly effective in treating short-term back pain.

  • Helps improve posture:

Sitting at a desk can result in bad posture over time and cause problems like neck pain, headaches, and migraines. Spinal Manipulation Therapy (SMT) can help reduce headaches and relieve neck and upper back tension, and it is also considered superior to the effects that medication can provide.

  • Helps athletes improve performance levels:

Professional athletes often hire chiropractors to relieve pain caused by sports injuries and help them recover faster. Performing intense physical activity while training can also cause exhaustion and physical stress on other body parts. Chiropractors may perform soft tissue therapies to reduce this stress and restore original mobility.

What are the potential side effects or risks of a chiropractic adjustment?

After a chiropractic adjustment, some individuals may experience mild soreness, fatigue, or a slight headache, typically short-lived.

In rare cases, chiropractic adjustments could result in more serious complications, including herniated discs, pinched nerves in the lower spine (cauda equina syndrome), and stroke. These risks are higher if someone untrained or unlicensed performs these adjustments. It’s essential to seek treatment only from a licensed and professional chiropractor.

What to expect at your first visit to a chiropractor?

The first visit to a chiropractor will involve evaluating your issues and developing a treatment plan. They will discuss your medical history, current issues, and what you hope to gain from chiropractic care. This session is an opportunity for you to discuss how you've been feeling physically, as well as any issues with your body that have hurt you.

Some of the things the chiropractor may discuss include:

  • Medications you may be taking
  • Past injuries or other health conditions
  • Exercise and dietary routine
  • Alcohol, tobacco, or drug use
  • Sleeping habits
  • Mental stressors

The practitioner will then conduct a physical exam to assess your range of motion, muscular strength, posture, and any other potential structural abnormalities. They may evaluate your spinal mobility and record vital signs like blood pressure. If necessary, they may order imaging tests such as magnetic resonance imaging (MRI) scans or an X-ray to better understand what's going on inside the body.

Based on the results, the chiropractor will create a treatment plan. On that day, the chiropractor can perform an adjustment or other therapy or schedule another appointment to begin regular therapy, which may include a combination of spinal adjustments and other treatments.

Depending on your diagnosis and goals, your chiropractor may recommend a series of treatment sessions. These subsequent sessions can be shorter, lasting around 20 minutes each, and occur a few times weekly. As your condition improves, the number of sessions may reduce.

Prevalence of chiropractors and musculoskeletal disorders in Selden, New York

Per The Bureau of Labor Statistics (BLS), the state of New York employed over 1,200 chiropractors as of May 2021. The New York-Newark-Jersey City (NY-NJ-PA) metropolitan area has one of the most employed chiropractors. However, this figure is significantly lower than comparable professions, such as physical therapists (16,000).

Arthritis is a common condition treated by chiropractors. Per data from the Centers for Disease Control and Prevention (CDC) in 2020, about 18.7% of all adults in New York had arthritis. The percentage of arthritis was higher in women (21.2%) than in men (15.8%). Among adults with arthritis in New York, an estimated 26.5% are obese, 28.2% have diabetes, and 37.1% have heart disease. In 2019, research showed that 29.3% of New Yorkers with arthritis also had severe joint pain. Almost 31.8% of all aged 18 and above with arthritis were also physically inactive.

A New York State Department of Health report indicated that 15% of the NYS Occupational Health Clinic Network (OHCN) patients aged 55 and above had problems related to the musculoskeletal system. Of all NYS OHCN patients, twice as many women (29%) were diagnosed with musculoskeletal disorders than men (11%). Nearly 73% of all women in the workforce were diagnosed with Carpal Tunnel Syndrome (CTS), and 72% had disorders of the muscles and tendons.

Data collated by the BLS in 2000 also showed that over 40,000 people working in the private sector were diagnosed with musculoskeletal problems and had lost work days. One-fourth of these issues included neck, shoulder, and upper extremities, and over 50% were of the back. Physical and ergonomic factors were responsible for one-fourth of all these exposures.
