What is Medicare?

Medicare is a federal health insurance program in the United States that mainly serves those 65 years of age or older. It covers specific medical services, some of which are chargeable to the insured. Broadly, it provides medical coverage to:

  1. Younger patients with specific disabilities
  2. People who are 65 or older
  3. People suffering from end-stage renal disease


Medicare coverage eligibility is subject to a number of qualifying conditions; however, it is generally available to anyone 65 years of age or older who has been lawfully residing in the United States for at least five years. As mentioned above, individuals who receive Social Security Disability Insurance (SSDI) and are under 65 years of age may also be eligible. 


Medicare operates and offers services similar to health insurance, but it does not entail any premium for its basic coverage. The differentiating factor is that, unlike private health insurance, Medicare is limited in its coverage of chronic illnesses and ongoing care. It also does not allow enrollees to extend their coverage to family members as is common in many private health insurance plans. 


What are the different categories under Medicare?

There are four types of Medicare coverage programs (Part A, B, C, & D) which are differentiated on the basis of the scope of offers and services each type provides. Part A and Part B form a part of what is called the “Original Medicare” and these are components of the basic coverage. 


Part A


Part A covers the medical expenses incurred at nursing facilities, hospitals, home health services, clinics, and hospices. However, this type of Medicare does not take into account custodial or long-term care under its coverage plan. For people who receive Social Security benefits, coverage is automatic under Part A. Most eligible people do not pay a monthly premium; however, non-eligible people may have to pay upto $505 monthly premium under Part A.


Part B


Medicare Part B pays for outpatient care expenses like doctor visits. It also covers ambulance transportation, mental health services, preventive treatment, and specific medical equipment. Most people pay the standard premium of $174.70 under Part B, which might be more depending on a person’s gross income as reflected in the IRS tax return. 


Part C


Part C (also referred to as Medicare Advantage) is available to those people who are eligible for Parts A and B. Most Medicare Advantage plans offer out-of-pocket costs and annual caps, and they may also offer benefits, such as hearing, dental, and vision care.


Part D


Medicare Part D provides additional coverage for prescription drugs. Medicare Part D enrollees who are enrolled in Part A or Part B may apply for subsidies to pay the price of prescription drugs that are not covered by Original Medicare coverage.

The MCP model recognizes primary care is the best place to start for value-based care payment models since it may avoid the need for expensive, specialized treatments by addressing health issues proactively and emphasizing preventive measures. A study published by the Health Affairs Scholar suggested that an emphasis on value-based primary care led to:-


  • less expenditure on claims by about 11.7%
  • 12.7% fewer emergency department visits
  • 16.6% lesser lab tests, and
  • fewer specialty drug prescriptions by nearly 21%


MCP aims to provide more value to both patients and providers, encouraging both to participate.
