Welcome to Mishaan Psychotherapy. I'm a Licensed Mental Health Counselor and Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with over ten years in practice, working with adults facing addiction and substance use, trauma and PTSD, anxiety, depression, compulsive behaviors, and the transitions of emerging adulthood. I offer online therapy to residents of New York, New Jersey, and Florida, and in-person sessions on the Upper West Side. Much of my work is with people who have already been to therapy — sometimes for years — and found that managing symptoms never quite reached what was actually driving the pain. I don't think therapy is only about the elimination of symptoms. I think of it as a catalyst for discovering a person able to live with more meaning, vitality, and freedom. Rather than education or behavior modification alone, my focus is on understanding the intricate ways you've learned to relate to yourself, to others, and to the world — and using our relationship as a place where those patterns can be seen, questioned, and changed. I draw on psychodynamic, relational, Internal Family Systems (IFS), mindfulness-based, somatic, and trauma-focused approaches, and I offer ketamine-assisted psychotherapy (KAP) and psychedelic integration for clients who want a deeper path. Don't expect one-size-fits-all. We'll find what fits you. Therapy with me is a warm, non-judgmental space. I offer a free 15-minute consultation to see whether we're a good match.

What should a new client know about working with you?

A few things worth knowing before we start: I work deeper than symptom relief. If you want a structured skills course or a worksheet-driven protocol, I'm probably not your best fit, and I'll say so honestly. My clients are usually people for whom surface-level approaches haven't held — and who are ready to look at the underlying causes. The relationship is part of the treatment, not just the setting for it. How you and I relate to each other over time often reveals the same patterns that show up everywhere else in your life. Noticing that together, in real time, is one of the most useful things therapy can offer. I'm not shocked by anything. As a CASAC-credentialed counselor, I've spent years with addiction, substance use, and compulsive behavior. You don't need to clean up your story before bringing it in, and you don't need to have already decided that abstinence is your goal in order to start. I offer ketamine-assisted psychotherapy. For treatment-resistant depression, trauma, anxiety, and addiction, KAP can open doors that talk therapy alone hasn't. It's not a quick fix, and it isn't right for everyone — we'd assess carefully before beginning. I also provide psychedelic integration for people who work with these medicines on their own; I don't prescribe or provide any substances. Fit matters more than filling a slot. If I'm not the right therapist for what you need, I'd rather tell you and point you toward someone who is. That's part of what the free consultation is for.

What is your typical process for working with clients?

A free 15-minute consultation. We talk about what brings you in and what you're hoping for, and you get a sense of me. No pressure to book. The first few sessions. We go over your history — family, relationships, substance use, trauma, previous treatment, what's worked and what hasn't — and get clear on what you actually want to be different. I'll share how I'm thinking about it and what I'd suggest. Ongoing work, usually weekly. Sessions are conversational rather than agenda-driven. We follow what's alive: what happened this week, what's happening between us, what's happening underneath. Over time we come to know the parts of you carrying pain, protection, or shame — and how they came to take on those roles. As that understanding grows, choices that once felt impossible start to open up. If we pursue KAP, the structure has three phases: preparation (building trust, setting intentions, reviewing history), the ketamine sessions themselves with me present throughout, and integration — the follow-up work that turns insight into lasting change. Integration is where the real value lives, and it's where I spend the most care. Ongoing review. We check in periodically about whether this is working. Progress isn't linear, and I'd rather name a plateau than pretend past it.

I'm in-network with Aetna, Cigna, UnitedHealthcare, Optum, and Oscar. If you're using insurance, I'd encourage you to confirm your mental health benefits, deductible, and copay directly with your plan before your first visit. For clients paying out of pocket, initial sessions are $175 (45 minutes), with ongoing sessions ranging from $140–$200 depending on session length and format. I hold a limited number of reduced-fee slots — if cost is the barrier between you and starting, please raise it during the consultation rather than ruling therapy out. If I'm out of network for your plan, I can provide a monthly superbill for reimbursement. Ketamine-assisted psychotherapy is billed separately from standard sessions, since KAP appointments run longer and involve a prescribing medical provider through Journey Clinical. Medical evaluation and medication costs are handled by that provider and are not included in my fees. I'll walk you through the full cost structure before you commit to anything.
