Getting to know Nathaniel Cilley

If you’ve been living with constant anxiety, stress, or self-doubt, you may feel like you’ve tried everything—or that nothing will ever truly change. I want you to know: lasting transformation is possible, and you don’t have to do it alone. As a psychotherapist certified in Rational Emotive Behavior Therapy (REBT) and Cognitive-Behavioral Therapy (CBT), I help clients break free from the patterns of anxious and limited thinking that keep them stuck. These approaches give you practical, proven tools to shift your mindset, calm your body, and feel more in control—so you can finally stop just “managing” symptoms and start living. For those ready to go deeper, I also offer: Manifestation Coaching – A powerful way to move beyond symptom relief and create the life you truly want. Here we blend psychological principles, quantum physics, and spirituality to help you align your thoughts, emotions, and actions with your ideal self. Ketamine Assisted Psychotherapy (KAP) – When anxiety or deeply rooted beliefs feel too rigid for talk therapy alone, KAP can open new pathways for healing. I guide you safely through preparation, the process itself, and integration, so that the breakthroughs you experience can be effectively integrated. Whether you’re looking for relief from overwhelming anxiety, a way to reconnect with your purpose, or a transformative tool for deep-seated challenges, you’re in the right place. I meet you where you are, and together we’ll choose the approach that best supports your healing and growth. My role is to walk alongside you, challenge you when needed, and provide both evidence-based strategies and heart-centered guidance. You’ll not only learn how to quiet the storm of anxiety—you’ll discover how to reclaim your confidence, your calm, and the vision of your dream life. You don’t have to figure this out alone. I offer a free 15-minute consultation to meet you where you are, and explore how we can create lasting change together.

What should a new client know about working with you?

I’m deeply passionate about helping people create real, lasting change in their lives. My foundation is in Rational Emotive Behavior Therapy (REBT) and Cognitive Behavioral Therapy (CBT) —a powerful, evidence-based approach that helps you identify, challenge, and shift the thoughts and beliefs that fuel anxiety, stress, and self-doubt. I believe in its effectiveness so much that I not only practice it, but I have also been certified in it to train and supervise other therapists in these modalities. At the same time, I also hold a strong belief in the power of manifestation and conscious awareness. I’ve seen how blending practical psychology with deeper spiritual principles helps people not only heal but also expand into lives that feel aligned, purposeful, and fulfilling. Clients often describe me as warm and empathetic, but also direct and honest. I’ll listen closely and meet you with compassion, but I won’t just nod and let you stay stuck—I’ll challenge your thinking, redirect you when needed, and hold you accountable to the changes you say you want. I believe in personal responsibility as the key to lasting change, and I’ll be right there beside you every step of the way. If you’re looking for someone who will encourage you, challenge you, and give you both practical tools and a deeper perspective—you’ll be in good hands. My goal is not just to help you feel better in the moment, but to help you get better so that you'll be able to manage your emotions independently.

What is your typical process for working with clients?

Our sessions are active, structured, and focused on helping you achieve a lasting transformation. We usually begin by checking in on the highlights and challenges of your week during the first few minutes, then we choose one specific problem to focus on together for the duration of the session. Through open-ended questions, I’ll help you uncover the underlying beliefs driving your anxiety or stress. From there, we’ll use practical exercises to challenge those beliefs—testing how true or helpful they really are—and explore new ways of thinking about the situation. Just as important, we’ll look at different behavioral responses you can try so you can break free from old patterns and cycles of avoidance, procrastination, indecisiveness, etc. You can expect our work to be collaborative, engaging, and goal-oriented. I’ll give you homework between sessions so you can practice the tools we discuss and build momentum toward change. The goal isn’t to keep you in therapy forever—it’s to equip you with the skills and insights you need to feel better, achieve your goals, and move forward in your life with confidence.

When I started this journey, I was struggling with depression and anxiety and often felt lost. Healing wasn’t easy, but Nathaniel’s patience and steady support made all the difference. His skill and genuine passion for helping others shine through in every session. Today, I feel stronger, grounded, and truly excited about the future.
Nathaniel is great! Always helps to deconstruct things going on in life and how to rationally work through it and adjust your thinking moving forward. He's a pleasure to work with and would recommend to all!
Nathaniel has had a highly positive impact on my mental well-being in a short span of time. I’ve worked with a couple of therapists previously, with mixed results, but I find that the support Nathaniel provides is exactly what I need. It’s the perfect blend of pragmatic advice and allowing enough room for me to express myself how I want. I can be quite scattershot when it comes to my thoughts and layers of thought, and as I discuss my challenges, he is very skilled at keeping up with the curve and continues to offer meaningful insights and advice.
Nathan is an amazing therapist. I worked with him for over 3 years. He is patient, listens to his patients, and offers constructive tools and methodologies to address mental health in a comforting and non-judgmental way. I have seen a significant improvement in how I handle stress triggers as a result of Nathan’s help and counseling
Having done therapy for the past 10 years, Nathaniel is probably the best therapist I had the pleasure of working with thus far. He has helped me overcome a major anxiety by providing me with very practical and methodical tools, which significantly improved my quality of life. Nathaniel is very understanding and easy to talk to, and for the first time ever made me feel comfortable to be 100% open with a therapist.
I worked with Nate for more than two years. He creates a very positive environment and he taught me a number of tools for dealing with anxiety and depression that have helped me substantially.
I worked with Nathaniel for many months. He is professional, kind, and a great listener. He provided me with the necessary tools and execises to navigate difficult times through my 20s and always made me feel comfortable and heard. I will continue to work with him through the years.
I am glad I was able to find this gem of a therapist. I suffered from anxiety and depression for years. Lucikly, I came across Nathaniel Cilley on zocdoc. On my very first visit, I felt that it was a right fit for me. His soothing, calming voice combined with his great listening skills made me feel comfortable and allowed me to open up and narrate clearly what I was going through. He geniunely helps his patients reach their best self. This is clear from the fact that he teaches his patients how to use REBT technique so it can be readily used whenever needed.
When I first sought out therapy, I knew I was in a deep, dark difficult place and I desperately wanted to feel like myself again. I had gone through these depressive episodes throughout my life, but didn’t understand what triggered them, how to deal with them or even what to call them. In our first meeting, Nate and I talked about what I wanted to get out of therapy, what that process would be like, and what that timeline might be. Through our sessions, I’ve gained clarity around what the underlying irrational beliefs that contribute to my anxiety and depression are, and also how to recognize them and manage them. Three years later I’ve accomplished all of the goals we initially set out and many more. I feel equipped to handle what previously felt like insurmountable challenges. Working with him was a major inflection point in my life. I am a much happier, stable, and productive version of myself because of the skills, techniques, and insights Nate has worked so diligently on with me.
Nate helped me work through some issues I had related to work and family. He shared personal experiences of his own and that made it easy for me to open up. He had a clear actionable plan that helped me tremendously and also helped me express what I was feeling. My partner's words after working with Nate for a few months: "I really think it worked, I can see the change in you". Thank you, Nate!
Nathaniel was great in all aspects of his service. He had great listening skills, always helped create a course of action, keeping struture to our sessions, and treatment. He always provided a fresh insight into situations and ways to help address them in the future. I would recommend Nathaniel to anyone that is looking to take toxic and negative thinking and turn it into productive thoughts in practice.
I have been working with Nathaniel for about 2 years now. I started seeing him due to depression and anxiety stemming from divorce, a geographical move, Covid-19 lockdown, and other issues. In the 2 years since we have been working together my mood and ability to address my anxiety and depression has seen a marked improvement. He has taken a different approach than my past providers and I cannot speak highly enough about Nathaniel as a mental health care provider. I am in a much better place mentally and it is thanks to the efforts and approach Nathaniel has taken.
Nathaniel helped me change my life considerably. I was anxious and depressed when I first started having these appointments. Nathaniel provided framework and guidance that helped me realized the causes of my anxiety/depression and then gave me language/framework on how to combat these symptoms. It is the first type of therapy that not only helped me feel better in the moment, but I actually feel that I AM better. I have developed the skills I need to continue on this journey, and I'm forever appreciative of Nathaniel and his work. He is kind, intelligent, patient, and supportive. I can't recommend him enough!
Having never been to therapy, I was nervous about being comfortable enough to be transparent. My fears were quickly put to ease working with Nathaniel to help with anxiety issues and maladaptive coping behaviors. He is kind, empathetic and helps to break down concepts into practical and actionable exercises to make lasting impact. He is also very honest that therapy isn't a quick fix, which is important to managing expectations with how therapy helps. I would highly recommend!
Nathaniel has been instrumental in my personal growth this year. This was my first time using REBT style therapy, which sets actionable goals and techniques I apply to everyday life. Our first session together was unlike any other therapist I have seen- our goal is that I won't need to see him every week forever. Unlike past therapists, I come out of these sessions feeling like I have the tools help me conscious decisions to shift my mentality and ultimately my life. Thank you Nathaniel!
I always wanted to visit a therapist, but I never was able to get myself to go. After working with Nate, I regret not talking to someone sooner. Nate is an attentive listener and provides great insights and feedback. He encourages me to think about situations from different angles and provides tools and techniques to try out after each session with have proved to be incredibly valueable. For those on the fence with seeing a therapist, I highly recommend a call with Nate.
I started seeing Nate for anxiety. he helped me address my anxiety by reexaming my beliefs in a more rational way. It helped me tremendously. I've climbed out of the anxiety spiral and feel more confident and safe. Highly recommend Nate for anxiety management.
I loved my 1st visit. Nate allowed me to feel comfortable & safe. It was easy to open up and begin the process of growing & learning new behavior. This has been one of the best decisions I've made.
Nathaniel's been extremely helpful in helping me deal with social anxiety and shame. He's good at helping me describe situations accurately, breaking down unhelpful beliefs and perceptions, and helping form healthy and believable new thought patterns. His analogies are especially sharp, and reveal the absurdities in some negative thoughts so quickly. I've had many therapists over 10 years, and there's been no other like him.
Words are difficult to express the amount of time and attention Nate spends on finding the root causes of hard fought personal issues. Being a skeptic and being long removed from speaking to a therapist, never having spoken to one in my adult life, I can honestly say I am the best version of myself after meeting for only 6 sessions. The tips and exercise of the brain in our sessions to look at life in an entirely different way has been the best thing I've done for myself, ever, all with the help of Nate.
Nathaniel has been incredibly helping in providing me the language and understanding to work through my personal beliefs and become more productive for the things I care about. His focus on rational and irrational beliefs resonates with me and has helped me thrugh several tough issues.
I have been suffering from severe anxiety and panic attacks for 11 years. I've been with at least six different therapists since the onset of my first episodes. It wasn't until I began therapy with Nathaniel that I began to show significant progress and my visit with him are extremely productive. Therapists can oftentimes appear dry and impersonal but that's not how it is with him. You get the sense that Nathaniel truly cares about helping you get better by equipping you with the tools to do so. Besides having the knowledge to help you, Nathaniel truly has the necessary compassion needed for patients like myself.
Nathaniel has been incredibly helpful, kind, and understanding through all of my sessions. From the very beginning, he made me feel comfortable and I have enjoyed working with him each week. I would highly recommend his services to anyone in New York looking to feel and get better!
I arrived for my appointment and Nathaniel was very warm and welcoming. Immediately, I felt comfortable and at ease which is very difficult for me with meeting anyone for the first time. His sincere & genuine nature made it very easy for me to be honest and comfortable in his presence.
Over the past few years, I struggled with anxiety due to some big life changes. Nathaniel has been instrumental in helping me understand my anxiety and its root causes. I never went to a therapist before Nathaniel, and I had my doubts. But every session is judgment-free. No matter what, I know he's always in my corner and will challenge me to be my best self. He has been and continues to be a great therapist and coach.
