Getting to know Stephanie Fierro

I am a Licensed Mental Health Counselor with over ten years of clinical experience supporting individuals, couples, and parents through anxiety, significant life transitions, and relational difficulty. . I am the founder and clinical supervisor of a virtual group practice, and I am licensed to provide telehealth across multiple states. My clinical approach is collaborative and evidence-informed: I work with clients to identify the patterns maintaining their distress, and to build practical, sustainable strategies for managing it. Treatment is structured and goal-directed while remaining responsive to what each client brings into the session.

What should a new client know about working with you?

I take an active, directive role in session. Clients can expect me to ask clarifying questions, offer clinical observations, and identify patterns as they emerge. I will also be direct with you about what I am seeing, including when it may be uncomfortable to hear, because I have found that clients make the most meaningful progress when the work is honest. Sessions are conducted virtually, which allows for consistency around demanding work schedules, caregiving responsibilities, and childcare constraints. I ask that clients come prepared to participate between sessions as well; the change most clients are seeking happens in the ordinary hours of their week, not solely in the therapy hour. If your presenting concerns fall outside my scope of competence, I will tell you and assist you in connecting with an appropriate provider. As a clinical supervisor, I maintain an active referral network and take that responsibility seriously.

What is your typical process for working with clients?

The initial one to two sessions are dedicated to assessment. I gather a clinical history, review current symptoms and functioning, and clarify what has brought you to treatment at this particular point. From this we establish an initial treatment focus and identify specific, observable goals. Ongoing sessions are typically weekly at the outset. Early work generally centers on stabilization and symptom management — sleep, functioning, acute anxiety, or immediate relational conflict — before moving into the longer-term patterns underneath. I integrate cognitive-behavioral and systemic approaches, adjusted to the presenting concern and to what a given client responds to. Couples and parenting work is structured around identifiable communication and behavioral patterns rather than general discussion. I revisit goals periodically to assess progress and, where indicated, adjust the treatment plan. As clients stabilize, we often reduce frequency to biweekly or monthly before concluding treatment, and I discuss termination and relapse prevention explicitly rather than allowing the work to taper off without a plan.
