Who are physical therapists?

Physical therapists (PTs), also known as physiotherapists or movement experts, are healthcare professionals trained and licensed to prevent, diagnose, and treat all abnormalities related to the body's physical functioning. These abnormalities can result from a traumatic incident, a disability, or a health condition.

Physical therapists assist patients in increasing their body's strength, range of motion, and mobility, avoiding additional injury or impairment, and enhancing their quality of life by designing specific exercise regimens for them. They practice in numerous settings such as private clinics, hospitals, nursing homes, rehabilitation centers, schools, outpatient centers, fitness centers, research centers, centers providing occupational therapy, and government agencies.

What kind of education and training do physical therapists receive?

Physical therapists in the United States undergo extensive schooling to become experts in their field. They gain a Doctor of Physical Therapy (DPT) degree from an accredited program. The degree usually takes about three years to complete. During their studies, they learn about various topics related to the body, such as biology, anatomy, and exercise physiology.

Most physical therapy programs need students to have a bachelor's degree before applying. Some programs offer a 3 + 3 format, where students spend three years studying undergraduate courses. The curriculum helps students focus on physical therapy before starting a three-year DPT program. Others may have direct entry programs for high school graduates, allowing them to start the DPT program after completing undergraduate requirements.

During their DPT program, students spend significant time in classrooms and labs, studying topics like anatomy and neuroscience. They also gain hands-on experience through clinical education, where they work with patients under the supervision of experienced physical therapists.

After becoming licensed, physical therapists can choose to specialize further by participating in clinical residency or fellowship programs. These programs offer focused training in specific practice areas, allowing therapists to gain more expertise in treating certain conditions.

Physical therapists can become board-certified clinical specialists through the American Board of Physical Therapy Specialties for those seeking additional recognition. While not required, this certification demonstrates expertise in a particular area of physical therapy practice and commitment to ongoing learning and improvement.

What are the specialties in physical therapy?

Some P.T.s go on to do a residency or fellowship in one of the physical therapy specializations

  • Orthopedic physical therapy: Physical therapists specializing in orthopedics diagnose and treat musculoskeletal injuries. This includes sprains, fractures, bursitis, tendinitis, etc. Some treatments they provide include mobility training, surgeries, joint mobilization exercises, strength training, and manual therapy.
  • Neurological physical therapy: Patients suffering from neurological illnesses such as cerebral palsy, Parkinson's disease, Alzheimer's disease, multiple sclerosis, and stroke can benefit from neurological physical therapy. This treatment focuses on improving muscle tone and movements, increasing the responsiveness of the limbs, dealing with balance issues, and enhancing strength.
  • Cardiopulmonary physical therapy: Physical therapy assists patients with ongoing heart or lung conditions or who have had surgery in these areas by enhancing their muscle strength, endurance, and stamina.
  • Geriatric physical therapy: Old age has many conditions that affect the body's functioning and mobility. These physical therapists specialize in reducing pain, increasing fitness levels, and improving mobility in older adults having osteoporosis, arthritis, incontinence, and hip or joint replacement surgeries.
  • Sports physical therapy: Physical therapists specializing in sports have complete expertise in preventing, diagnosing, and treating all injuries seen in athletes, such as sprains, knee injuries, fractures, dislocations, tennis elbow, ankle injuries, and pulled muscles. They help the athletes regain strength and body function following an injury and teach them exercises to prevent such complications.
  • Vestibular physical therapy: People with problems in the inner ear have balance issues. Physical therapy in such patients helps restore balance and coordination.
  • Wound care physical therapy: With the help of compression therapy, manual therapy, and electrical stimulation, physical therapists help patients who have recently suffered trauma. Regularly followed exercises help improve blood circulation, increase oxygen flow to the affected area, and speed up healing.
  • Pediatric physical therapy: Pediatric physical therapists receive special training to prevent, diagnose, and treat conditions affecting the kids. Some examples include spina bifida, developmental delays, torticollis, cerebral palsy, and other musculoskeletal conditions. They work closely with infants, children, adolescents, and their families to recommend exercises that decrease pain, improve mobility, and prevent or reduce permanent disability. They help kids learn the tricks of functioning independently everywhere they go.

Is occupational therapy and physical therapy the same?

Occupational therapy and physical therapy are similar as they help people improve their daily lives, but they focus on different things. Occupational therapists help people become better at doing everyday tasks. They focus on activities like dressing, cooking, and driving. O.T.s work closely with individuals to set goals and find ways to make these tasks easier.

While occupational therapy focuses on daily activities, physical therapy concentrates on improving movement. Both types of therapy aim to enhance quality of life and help individuals reach their goals.

How does physical therapy assist in treating life-threatening diseases?

Physical therapists primarily treat conditions that affect a patient's physical movement and mobility. However, physical therapy is now widely used to assist in various cases, ranging from minor injuries to complex diseases and disorders. Here are some severe illnesses that can benefit from physical therapy:

  • Chronic lung diseases: This includes asthma, lung cancer, interstitial lung disease, lung volume reduction surgery, pulmonary hypertension, and cystic fibrosis. People with these long-term lung disorders have breathing issues while performing daily tasks and need to learn new skills and exercises that help them function better.

Pulmonary rehabilitation is the name of the therapy used, and it entails a team of medical specialists, with physical therapists playing an essential part. After an initial assessment, patients with chronic lung disorders receive a customized program consisting of a daily exercise regimen and diet plans. Practicing the recommended exercises helps these patients deal with their shortness of breath and improves their quality of life.

  • Parkinson's disease: It is a brain disorder resulting in movement difficulties. Physical therapists help patients regain their strength, balance, and mobility. Depending on the patient's specific requirements, each workout program is unique.

Amplitude training exercises such as LSVT BIG training involve exaggerated movements in arms and legs, which helps the patients slow down the progression of hyperkinesia(excessive restlessness) in their muscles. Gait training exercises help in improving balance while walking. Stretching exercises help reduce stiffness in the muscles. Resistance exercises using resistance bands, dumbbells, and water help build strength.

  • Cancer: Doctors treat this condition using aggressive treatments that often lead to physical, cognitive, and psychological problems for the survivors. Cancer rehabilitation is a specially designed program to help these patients stay active and independent, reduce the side effects of the treatment, and improve their quality of life.

Physical therapist-guided exercise helps cancer patients with physical issues such as weakness, loss of strength, flexibility, and body motion. Additionally, these exercises aid in overcoming mobility problems, such as getting up from the floor, walking, ascending stairs, showering, and dressing. Oncology physical therapists are known to treat cancer patients.

What can you expect from physical therapy?

You may require physical therapy if you have difficulty moving due to decreased strength, range of motion, or pain. When choosing the best physical therapist near you, look for an ideal physical therapist who is patient, non-judgmental, and answers all your questions.

Before the appointment, write down important notes about your problem so you remember them. Your physical therapist may ask you a couple of questions, including:

  • How and when did you notice the problem?
  • What is it like to climb the stairs or perform other daily activities?
  • Is there something that makes the symptoms better or worse?

Carry a list of your medications and inform the doctor about any past chronic health problems or surgeries. After discussing your condition, the physical therapist near you will perform an examination. Typical measurements include functional mobility, range of motion, strength, balance, and neurological screening tests.

Choose comfortable clothing as you may have to move around. After your examination, the physical therapist will have a clear idea of your condition and can discuss a treatment plan with you. It can include techniques such as hands-on exercises, electrical stimulation, and ultrasound. Your physical therapist will prescribe at-home activities that will benefit you.

Depending on your condition, the doctor will also recommend a frequency of appointments. They will also discuss the outcomes you can expect from physical therapy. The benefits of physical therapy are:

  • recovery from injuries or trauma
  • improved mobility and balance
  • avoiding surgery
  • recovery from stroke or paralysis
  • fall prevention
  • pain management with reduced need for opioids

How to boost your strength and coordination at any age

As we age, it’s easy to believe that slowing down physically is inevitable. But staying active is crucial—even more so as we get older. Regular exercise helps counter the natural changes in hormones and other bodily processes that can otherwise lead to decreased strength and coordination.

The great news? You can build and maintain strength and coordination at any age with the right approach!

Here's how to get started:

  1. Embrace aerobic exercise: Aim for at least 30 minutes of brisk walking, jogging, biking, swimming, or aerobic classes five days a week. Cardio exercises keep your heart healthy, boost endurance, and improve overall energy levels.
  2. Add strength, balance, and flexibility workouts: Dedicate two hours per week to activities like yoga, tai chi, Pilates, or weightlifting.
  3. Practice your favorite sports: Golf, tennis, basketball, or any sport you love can help maintain coordination and keep you engaged. Learning new skills and honing old ones can stimulate both body and mind.
  4. Seek expert guidance: Trainers, coaches, and physical therapists can teach you safe techniques and help tailor your routine based on your needs.
  5. Address health issues: If you have any conditions that limit movement, like arthritis or balance disorders, work with your doctor to manage them. This management will help you stay as active as possible.
  6. Nourish your body: A Mediterranean diet that is rich in nutrients and vitamins from fish, olive oil, fruits, vegetables, and whole grains can fuel your muscles and brain, boosting energy and recovery.
  7. Prioritize sleep: Quality sleep is a mandate for recovery and coordination. Aim for 7-9 hours a night to recharge your body and mind.

With dedication and proper steps, staying active can transform how you feel, regardless of age. Start today and invest in your strength and coordination for years to come!
