AFTER YOU BOOK: Complete these steps to avoid cancellation or being switched to self-pay. 1. Check your email for instructions to set up your Thriveworks account. Within your Thriveworks’ account, sign consent forms and add a valid credit card, which will be automatically charged for copays, fees, post-insurance balances, and self-pay amounts. 2. If using insurance, upload clear photos of the front and back of your insurance card. 3. Expect a confirmation call from our team to verify your information and answer questions. CANCELLATION POLICY: Provide 48 hours advance notice to avoid the $135 late cancellation fee. My name is Rachel and I love providing skills based care to individuals, couples and families.
