Getting to know Angela Lisak

***Talk therapy only. I do not prescribe medication.*** ***Summer schedule starts May 21 and ends August 20. This will limit weekend availability.*** If you do not find a time slot that works for you, please message me, and I will do my best to find a time that fits both our schedules. Do you feel stuck or like you have hit rock bottom? Do you feel no one understands or can help you? Do you have difficulty opening up? If you answered “yes” to any of these, you have come to the right provider! I am a licensed clinical social worker and a licensed clinical addiction counselor. I earned my BA in Psychology from Purdue University (2003) and my Master’s in Social Work from Indiana University (2006). I have been practicing since 2006. My experience includes 4 years in private practice settings, 12 years working with DCS/probation clients, and 7 years in the jail setting, including 5 years as clinical supervisor. Along with private practice, I currently serve as the clinical director of a substance abuse rehabilitation facility. I have worked with groups, couples, and individuals facing anxiety, depression, PTSD, bipolar disorder (with and without psychotic features), OCD, ADHD, suicidal ideation, crisis intervention, gender issues, parenting challenges, stress, self-esteem, self-worth, self-respect, and many more. I have a passion for working with those struggling with opiate addiction. I believe that working with substance issues requires a special kind of person. Substances often act like umbrellas shielding us from the raindrops of trauma. To remove the umbrella, we must address those raindrops. I will work with you to accomplish this!

More information on payments and coverage

Payment is due after each session and will be charged with your card on file for any portion of patient responsibility. Receipts will be provided at the time of the charge. This typically takes place 2-4 weeks after the session, once your insurance has had a chance to process the claim. We try our best to verify eligibility, but it is ultimately your responsibility to check coverage and out-of-pocket costs for services. For out-of-network visits, you will be billed at the provider’s self-pay rate, and we can provide an invoice for you to submit to your insurance for potential out-of-network reimbursement based on your plan’s benefits.

Easy to setup and didn’t take much effort. I like that I could take a picture of my insurance card and it was autofilled and let me know right at the beginning a list of doctors that were in my network and at the bottom of there list were ones that weren’t in network. Very organized too!
