After you book with me through Zocdoc, you will receive an email from our office through Simple Practice with paperwork to complete. You MUST finish all paperwork BEFORE your scheduled appointment to prevent the need to reschedule. A payment method will be collected during the process. This payment method will be used to cover any insurance co-payments, service fees, or missed-appointment/late-cancellation fees. A $50 fee will apply if you miss or cancel a session late (less than 24 hours before your appointment). The missed appointment and late cancellation fee may be waived if you are ill, unavoidably detained, or reschedule within the same week as your original appointment.
