I was working with Shakeiva for a few sessions. We were also trying to fill out FMLA for my job, but we had a miscommunication and the first form said the incorrect thing and Shakeiva did not even fill it out completely. So I had to return it and try to get it completely filled out with the right information. Shakeiva was upset because it is taking away from her time getting paid to fill it out. So she created an appointment (yet shakieva just booked it and never confirmed the time worked for me) and I have time stamps showing I was waiting for almost 20 minutes and she had not joined. Then I had to leave which shows in emails I sent shakieva. Then I received an email stating that “I” never showed up and that she just updated the form , and that if I didn’t like it, to find a new provider without making another appointment (knowingly I had a deadline and could not find another provider on time for FMLA). Which resulted in me losing my job. I really hope Shakieva can improve from this.
