Role of a nurse practitioner in medicine

Nurse practitioners (NPs) are healthcare experts who provide medical care in various settings. In addition to urban areas, NPs also serve patients of all age groups in rural areas, where healthcare is not easily accessible.

Nurse practitioners intervene in three care situations: acute, primary, and specialty care. They have different responsibilities depending on their specialty, healthcare setting, and the state's practice environment. Those working in a full-practice state can work independently, while others in restricted states may only work under the supervision of a physician.

Some everyday duties of nurse practitioners include gathering patient medical histories, ordering diagnostic tests, diagnosing acute and chronic medical conditions, injuries, and illnesses, and designing effective treatment plans for their patients. Certain nurse practitioners may also be allowed to prescribe medications. They can also write referrals to other specialists if the situation demands it.

In addition, nurse practitioners focus on routine and preventative healthcare services. These include immunizations, regular checkups, annual physicals, pelvic exams, birth control counseling, and health screenings. They also provide lifestyle and nutritional counseling to help patients improve their health and well-being.

Where do nurse practitioners work?

Nurse practitioners gain valuable experience during their training by working in diverse settings based on their specialties, allowing them to care for various patient needs. NPs practice in both inpatient and outpatient environments. Here are some ordinary workplaces where NPs work:

  • Private practice clinics: Nurse practitioners often collaborate with physicians, surgeons, and technicians in hospitals or private clinics. You may encounter them in offices specializing in dermatology, cardiology, pediatrics, gynecology, and family medicine.
  • Emergency rooms and urgent care centers: In these fast-paced environments, NPs provide immediate care for various illnesses and injuries and perform emergency procedures like intubation. Additionally, NPs work in urgent care centers in malls, pharmacies, or stores, addressing minor injuries and health concerns.
  • Schools: NPs or certified school nurses conduct routine health checkups and order tests for students in compliance with Department of Education guidelines. They may also educate teenagers on topics like sexually transmitted diseases (STDs) and sports injuries.
  • Community clinics: These clinics operate outside traditional hospital settings in small to medium-sized cities. Community health NPs offer general health screenings, family planning advice, and education on nutrition and women's health.
  • Rehabilitation and substance abuse treatment centers: Some NPs specialize in treating addiction disorders and assisting patients with detoxification. In rehab centers, they provide assistive devices, monitor patient progress, and aim to prevent hospital readmissions.
  • Nursing homes and hospice care: NPs care for older adults in nursing homes by monitoring vital signs, administering medications, and managing treatment plans. They also provide hospice care, addressing the physical/emotional/psychological needs of terminally ill patients and ensuring comfort during their final days.

What is the education and training of nurse practitioners?

To begin practicing, nurse practitioners must complete a four-year-long bachelor's degree in nursing (BSN) and become registered nurses (RN). They should then further their education by obtaining a Doctor of Nursing Practice (DNP) or a Master of Science in Nursing (MSN) degree, which typically requires an additional two to five years of study.

Nurse practitioners can choose various specialties during their education and training, including adult gerontology care, pediatrics, family medicine, etc. Once they pass the required exams to obtain their board certification and state licensure, they can start their professional practice as per applicable state laws.

Kinds of nursing subspecialties

During their master's program, nurse practitioners focus on a particular area within their practice to become experts. They may choose to subspecialize in the following fields:

  • Family medicine: Most nurse practitioners, known as family nurse practitioners (FNP), specialize in this field. They provide comprehensive primary and specialty healthcare services to patients of all ages. This includes conducting annual physicals, maintaining patient records, and detecting and treating acute and chronic conditions, illnesses, and injuries.
  • Neonatal medicine: Neonatal nurse practitioners (NNP) care for high-risk neonates, infants, and toddlers until they turn two. They practice in labor and delivery units, emergency departments, and neonatal intensive care units (NICUs). Nurses who are experts in neonatal medicine can manage low birth weight, premature complications, heart abnormalities, or other long-term health conditions.
  • Pediatrics: Nurse practitioners from this specialty take the initials PNP after their name. They are experts at providing primary and preventative care for patients from infancy through adolescence. Their duties include assessing growth and development, diagnosing and treating common illnesses, conducting annual health exams, and administering vaccinations.
  • Pediatric acute care: Their initials are CPNP-AC, meaning certified pediatric nurse practitioner - acute care. They care for pediatric patients suffering from acute, chronic, or critical care situations.
  • Adult-gerontology primary care: Nurse practitioners in this field are known as adult-gerontology nurse practitioners (AGNPs). They focus on providing comprehensive end-of-life healthcare to all adult patients.
  • Adult-gerontology acute care: An adult gerontology acute care nurse practitioner, shortened as AG-ACNP, improves the health of adult patients experiencing acute, chronic, and critical conditions and prevents future complications.
  • Psychiatry and mental health: These nurse practitioners, called PMHNPs, address their patients' mental health needs using medications and psychotherapy.
  • Women's health: A WHNP or women's health nurse practitioner provides complete gynecological and obstetric care to women throughout their lifetime. Their practice includes contraceptive counseling, diagnosing and treating gynecological conditions, cancer screenings, well-woman exams, pap tests, and more.

How do nurse practitioners differ from doctors?

At first glance, nurse practitioners and doctors have similar responsibilities. The primary difference between the two professions lies in practitioners' training. Becoming a nurse practitioner can take six to eight years, while becoming a fully licensed physician requires 10 to 14 years. Thus, nurse practitioners can start their careers earlier than doctors.

Another difference is the scope of practice. It varies for nurse practitioners from state to state. However, no such rules apply to doctors. Doctors are licensed to practice independently, irrespective of the state, and can work without supervision from another physician. Some states permit nurse practitioners to practice independently, while others make it mandatory to work with physician oversight. Additionally, a doctor's license is associated with greater authority, responsibilities, and privileges than a nurse practitioner's.

How are nurse practitioners different from physician assistants?

Physician assistants (PA) and nurse practitioners have the same healthcare role. They have many similarities, which can make it confusing to distinguish between them. However, while both diagnose medical conditions and treat patients, there are some differences in their roles, which are as follows:

  • Models of practice: Physician assistants attend medical school and train using the medical model. They treat various disorders based on their biological and physiological characteristics.
  • Nurse practitioners pursue nursing programs that follow the nursing model. A nursing model underscores how the diagnosis affects the patient overall. Nurse practitioners assess human aspects that may change due to the diagnosis.
  • Education: The main difference between an NP and a PA is their education. NPs attend nursing school, while PAs attend a medical school or center. A physician assistant is an advanced practice practitioner with a Master Of Physician Assistant Studies (MSPAS) degree licensed to perform many of the same clinical services as physicians in primary care settings.
  • Differences in specializations: Nurse practitioners can practice in various nursing specializations while earning certificates for the required fields. They must complete formal coursework and meet the license requirements to change their specialty certification from neonatal to family nurse practitioner. Once licensed, PAs can switch specialties without recertifying or undergoing further job training.
  • Scope of practice: Nurse practitioners work in many places, such as hospitals, doctors' offices, clinics, camps, schools, and businesses. In many states, NPs can practice independently, diagnose and treat illnesses, and prescribe medication.

Physician assistants usually work in doctors' offices and hospitals. Although they have some independence, in most states, they need to work under a doctor's supervision to practice. They can provide medical services but under a doctor’s oversight.

Advantages of consulting a nurse practitioner

Besides being excellent healthcare providers, nurse practitioners also play a role in health education, research, administration, and advocacy. They participate in community assessments, stay informed about patients' needs, and engage in public health policy activities to promote overall wellness, meet health goals, and build a healthier nation.

Nurse practitioners offer high-quality, affordable care to a variety of patients. Their vast experience and knowledge help communities adopt better health choices. It, in turn, helps reduce healthcare costs as patients frequenting nurse practitioners may face fewer health complications.

Compared to doctors, nurse practitioners are readily available in both urban and rural areas. They help tackle physician shortages nationwide by providing immediate care to patients. They facilitate longer consultations for every patient, thus giving them enough time to address all their concerns. It helps increase patient satisfaction and build long-term relationships for ongoing care and support throughout their lifetimes.

Statistics on physician shortages and nurse practitioners in Maryland

As stated in a 2022 American Association of Nurse Practitioners article, Maryland has given nurse practitioners full practice authority. This means that they can practice independently under the regulatory jurisdiction of the state board of nursing and run a private practice.

The U.S. Bureau of Labor Statistics published the Occupational Employment and Wages report in May 2023. In that year, there were about 5,180 nurse practitioners in the state. A Projections Central report estimates around 5,820 new nurse practitioners between 2020 and 2030, with approximately 420 average annual openings.

As per the Robert Graham Center's analysis of the physician workforce in Maryland in 2018, the state had around 4,811 primary care physicians (PCPs) in direct patient care, of which about 2,176 were general internists, another 1,249 were pediatricians, 1,213 were family physicians, and only around 101 were geriatricians.

On a per capita basis, there were nearly 79 PCPs per 100,000 persons in Maryland. Around 48% of the family physicians were above 55 years old. Approximately 9% of the total population of Maryland lived in underserved counties where the people-to-PCP ratio was more than 2,000:1. Many factors, such as aging physicians and lack of healthcare in underserved areas, contributed to the physician shortage and increased demand for nurse practitioners.
