Stefan Heise, LCSW

Psychotherapist
NY
I’m a Licensed Clinical Social Worker (LCSW) with nearly two decades of experience helping clients navigate anxiety, trauma, substance use, and relationship challenges. My approach combines evidence-based methods like cognitive behavioral therapy (CBT) with a process I call the Signal Circle — a way of understanding emotions not as problems, but as signals that point toward something meaningful. Many of my clients manage work, relationships, and responsibilities — yet still feel overwhelmed, reactive, or emotionally disconnected underneath. They may notice strong emotional responses, repeating patterns, or habits like overthinking or numbing — and want to understand what’s really going on beneath the surface. In our work together, we slow things down and use those reactions as data, tracing them back to earlier experiences, unmet needs, or internal narratives that deserve attention. The goal is not just symptom relief, but emotional clarity — the kind that helps you move forward with more awareness, boundaries, and connection to yourself. I also work with clients who are exploring their relationship with substances. My approach to recovery is respectful, collaborative, and grounded in harm reduction. Whether you’re considering change or working to maintain it, I’ll meet you where you are. My work is LGBTQ+ affirming, trauma-informed, and client-centered. I provide a thoughtful, nonjudgmental space to explore what you’re feeling — and why. Whether you're new to therapy or returning to it, you're welcome here.

What should a new client know about working with you?

I approach therapy with warmth, clarity, and respect. My goal isn’t to “fix” you, but to help you better understand what your emotions are trying to tell you. We'll slow things down, get curious about patterns, and use both practical tools and deeper insight to support meaningful change. I’ll meet you where you are — whether you’re feeling overwhelmed, stuck, or just ready to look inward. You don’t need to have everything figured out. We’ll work collaboratively, at your pace, to explore what’s happening underneath the surface and build toward more emotional clarity, self-trust, and connection.

What is your typical process for working with clients?

Our first few sessions focus on getting to know you — not just your symptoms, but your story, strengths, and goals. I ask questions to understand what’s bringing you in, what’s been working (or not), and what you hope will change. From there, we work collaboratively to clarify emotional patterns, understand what’s happening underneath your reactions, and explore what those patterns might be trying to protect or signal. I use a blend of structured tools (like CBT and TEAM-CBT) and deeper reflection work, including a process I call the Signal Circle — helping you turn emotional reactions into insight and growth. Sessions are active, warm, and focused. Sometimes we’ll practice new skills, sometimes we’ll explore past experiences, and often we’ll pause to notice what’s happening in the moment — because that’s often where the most important work begins.
